摘　　要：目的测定妊高征(PIH)患者胎盘组织中IFN-γ和IL-4 mRNA的表达水平,探讨两型细胞因子在妊高征发病中的作用。方法选择2006年10月～2007年10月在我院产科剖宫产分娩的妊高征患者40例。选择同期20例正常妊娠的孕妇作为对照。用RT-PCR法检测胎盘组织中IFN-γ和IL-4 mRNA的表达水平。观察比较妊高征患者与正常妊娠妇女胎盘组织中两类细胞因子IFN-γ和IL-4的表达差异与变化。结果①妊高征患者胎盘组织细胞中IFN-γ mRNA表达水平显著高于对照组孕妇的表达水平,其中重度妊高征患者表达水平增高极为显著(P<0.01)。②妊高征患者胎盘组织细胞中IL-4 mRNA表达显著低于对照组(P<0.01)。结论妊高征患者胎盘组织中Th1型细胞因子(IFN-γ)mRNA表达增高,而Th2型细胞因子(IL-4)mRNA表达降低,两者的变化与妊高征病情显著相关,考虑Th1/Th2型细胞因子的失衡可能是妊高征的发病机制之一。Objective To investigate the variation of cytokines interferon-γ （IFN-γ） and interleukin-4 （ IL- 4） in maternal placenta in pregnancy induced hypertension syndrome（PIH）. To discuss the role of cytokine and immunologic imbalance in the pathogenesis of PIH. Methods Forty patients with PIH who had cesarean section delivery （ CS ） during 2006 to 2007 in obstetrics department were choosen as study group and 20 normal pregnant women as control group. The study group was divided into three groups ： mild PIH patiens （ 10 cases）, moderate PIH patients （ 20 cases） and severe PIH patients（ 10 cases）. RT-PCR was used to measure the level of IFN-γ and IL-4 in placenta of all. Results （1）The levels of IFN--γ mRNA in placenta in PIH patients were significantly higher than those in normal pregnancy. In severe PIH,IFN-γ mRNA level was significantly higher（P 〈0.01）. （2）The levels of IL-4 mRNA in placenta in PIH patients was significantly lower than those of in normal pregnancy （ P 〈 0.01 ）. Conclusions In PIH patients, the level of Thl type cytokine（IFN-γ） increased while Th2 type cytokine（IL-4） decreased,these variations were correlated with the development of PIH. These changes of the cytokine levels may be a kind of important mechanism in the pathogenesis of PIH.
