摘 要:目的:探讨胃组织COX2在腹部火器伤肠管穿透后的表达及与血中内毒素水平、胃黏膜病理变化的关系。方法:健康长白仔猪42头随机等分为对照组和伤后1、2、4、8、12和24h组,实验组建立腹部火器伤肠管穿透模型后,用免疫组织化学图像分析法测定各组胃组织中COX2活性;采用显色基质鲎试剂法检测各组血浆内毒素水平;光学显微镜下观察各组胃组织形态学变化。结果:伤后各组胃黏膜COX2的表达明显高于对照组;实验组各组血浆内毒素水平均明显高于对照组(P<0.05),于伤后8h达高峰;伤后各组胃组织出现逐渐加重的胃黏膜下充血、水肿,12、24h组出现炎细胞浸润、浆膜下出血。结论:腹部火器伤肠管穿透后胃组织中COX2的表达增强,同时胃损伤性变化随时间延长而加重,提示COX2在腹部肠管火器伤后胃损伤过程中可能起重要作用。Objective: To investigate the changes of gastric tissue COX2 after intestinal perforations due to abdominal firearm wound. And to study its relationship with plasma endotoxin levels and pathological change of gastric tissue. Methods: A tota of 42 Chang-Bai piglets were assigned randomly into 7 groups: control group and wounded 1, 2, 4, 8, 12, 24 hours group.The model of intestinal perforations due to abdominal firearm wound was established in wounded groups. Gastric tissue COX2 activity was measured with immunohisto- chemical staining and image analysis in all groups. The plasma endotoxin levels were measured by chromogenic limulus ameboid lysate test. The alterations of gastr c tissue were observed under light microscope in all groups. Results The expressions of COX2 of gastric tissue in wounded groups were significantly increased compared with control group (P〈0.05). Plasma endotoxin levels of the experimental group were significantly higher than that of control group (P〈0.05), and the peak appeared from 8 h to 12 h after injury. The experimental group appeared gradually aggravated gastric mucosa hyperemia and edema under light microscope, Inflammatory cell infiltration and subserous hemorrhage appeared in 12 and 24 h group. Conclusion: The expression of COX2 in gastric tissue is increased while the change of gastric injury is aggravated with prolongation of wound after intestinal perforations due to abdominal firearm wound, which suggests that COX2 may play an important role in gastric injury of pigs.
