46例女性强直性脊柱炎临床分析

摘　　要：目的:探讨女性强直性脊柱炎患者的临床特点。方法:回顾性分析华西医院风湿免疫科2003年1月1日至2009年1月1日住院收治的女性强直性脊柱炎患者住院病历共计46例,抽取与女性患者入院时间及病程大致相等的男性病历40例,记录发病年龄、病程、首发症状、临床表现、实验室检查,并对两组数据进行比较,调查女性患者中血小板计数与血沉、C-反应蛋白、球蛋白的相互关系。结果:发病年龄女性组为(30±11)岁,男性组为(24±7)岁(P=0.003);首发症状、主要临床表现、关节活动受限和病情活动指标在两组无统计学差异(P>0.05)。相关分析显示,血小板计数与C-反应蛋白、血沉、球蛋白分别存在较好的正相关关系(C-反应蛋白:r=0.608,P=0.000;血沉:r=0.558,P=0.000;球蛋白r=0.411,P=0.005)。女性组中HLA-B27阳性患者与阴性患者的年龄差异无统计学意义(P>0.05)。结论:女性强直性脊柱患者的发病年龄大于男性,患者的血小板计数有助于评价病情活动。Objective：To discuss clinical characteristics in female with ankylosing spondylitis（AS）.Methods：The female AS cases were reviewed who were treated in in-patient Department of Clinical Immunology in West China Hospital since 1 January 2003 to 1 January 2009.The age at onset,course of disease,first symptoms,clinical manifestations and laboratory test results were recorded,then were compared with those of 40 male AS cases which roughly had the same admission time and course of disease.Correlation between the platelet count（PLT） of female patients with C reactive protein（CRP）,erythrocyte sedimentation rate（ESR） and globulin were analyzed.Results：The average age at disease onset was（30±11） years in women and（24±7） years in man（P=0.003〈0.05）,and there were no significant differences in first symptoms,mainly clinical manifestations,joint activity limitation and indices of disease activity between the two group.Correlation analysis manifested that there was a positive relationship between PLT and CRP,ESR,globulin（CRP：r=0.608,P=0.000〈0.01;ESR：r=0.558,P=0.000〈0.01;globulin：r=0.411,P=0.005〈0.01）.No significant difference in age at onset was found between HLA-B27 positive cases and HLA-B27 negative cases in female group.Conclusion：The age of disease onset is younger in men than in women.The PLT is proven to be helpful in evaluating the disease activity in AS patient.
