食管鳞癌中周期素D1和生存素的表达及预后价值

摘　　要：目的探讨细胞周期素D1(cyclin D1)和生存素(survivin)的表达与食管鳞癌临床病理因素和预后的相关性。方法应用免疫组织化学方法检测80例食管鳞癌中周期素D1和生存素的表达。结果细胞周期素D1和生存素阳性率分别为61.25%和65.00%。细胞周期素D1和生存素的表达均与淋巴结转移、TNM分期呈正相关(P<0.05)。生存素还与癌组织学分级有关(P<0.05),且与细胞周期素D1表达亦呈正相关(P<0.01)。多因素(Cox比例风险模型)生存分析显示食管鳞癌细胞周期素D1和生存素均是独立的预后因素(P<0.05)。结论细胞周期素D1和生存素均在食管鳞癌中高表达。均是影响预后的主要因素。Objective To investigate the expression and significance of cyclin D1 and survivin in esophageal squamous cell carcinoma（ESCC）. Methods The expressions of cyclin D1 and survivin in 80 patients with ESCC were detected by immunohistochemistry. Results The positive rates of cyclin D1 and survivin expressions were 61.25% and 65. 00%, respectively. The positive rates of cyclin D1 and survivin were positively correlated with lymph node metastasis and clinical stage （P〈0. 05）. The positive rate of survivin expression was positively correlated with the degree of ESCC differentiation （P〈0. 05） and that of cyclin D1 （P〈0. 01）. Multivariate Cox proportional hazards analysis showed that cyclin D1 and survivin were the independent factors for predicting the prognosis. Conclusion Cyelin D1 and survivin are higherly expressed in ESCC, which arc the independent factors for predicting the prognosis.
