摘　　要：目的:探讨具多次宫腔内人工授精(IUI)史者受精方式的选择。方法:比较≥3次IUI史者(A组,n=65)与无IUI史者(B组,n=65)的IVF结果及hCG注射日E2、LH值;对同期的IVF全部卵子受精失败者(C组,n=37)再按有无IUI史分为C1组(≥3次IUI史,n=8)、C2组(无IUI史,n=29),进行内分泌分析,并对他们下次的ICSI结果进行追踪、比较。结果:IUI史并不影响IVF的妊娠率,仅降低种植率;A组hCG注射日E2较低,而C组有较高的E2及LH值,C组中20例(54.1%)为继发不孕;但C1组62.5%(5/8)为原发性不孕。结论:多次IUI史并不是选择ICSI的唯一理由;原发不孕加多次IUI史有较高的受精失败风险;内分泌环境在受精中起重要的作用。Objective： To analyze the effect of repeated IUI on the results of IVF and to demonstrate the type of insemination. Methods： A total of 65 patients who had more than three IUI history were included in group A; anther similar 65 patients who had no IUI history were included in group B; and 37 patients who had completely failed fertilization were included in group C, among them, 8 patients with IUI history were included in group C1 and 29 patients without IUI history were included in group C2. IVF results and E：, LH in hCG injective day were compared among three groups. The results of next ICSI were compared between group C 1 and C2. Results： There was no statistically significant difference in pregnancy rate between group A and group B, but group A had lower implantation rate than group B; E2 in hCG injective day was the lowest in group A and group C had the highest E2 and LH. There were 20 cases （54.1%） of the secondary infertility in group C; and 62.5% （5/8） was the primary infeitility in group C1. Conclusion： The IUI history is not the only cause to choose ICSI. Patients with history of primary infertility and repeated IUI have high risk of completely fertilization failure; the endocrinological environment plays an important role in fertilization.
