摘 要:目的:通过检测子痫前期患者及正常孕妇胎盘组织中Ang-2及HIF-1α的表达,探讨其与子痫前期发病的关系。方法:选择子痫前期患者52例,并随机选择正常晚期妊娠妇女20例为对照组。两组胎盘组织HE染色高倍镜下计数绒毛滋养细胞层增生细胞结节数和绒毛间质的血管数,采用免疫组织化学方法检测胎盘组织中Ang-2及HIF-1α的表达情况。结果:子痫前期患者胎盘绒毛滋养层细胞增生结节数与正常晚孕组比较数量明显增多,绒毛间质血管密度显著下降(P<0.05)。子痫前期组Ang-2在胎盘组织中的表达明显低于正常对照组(P<0.05),而HIF-1α的表达显著高于对照组(P<0.01)。HIF-1α的表达水平与滋养层细胞增生结节数呈正相关(rs=0.794,P<0.05),Ang-2表达与绒毛间质血管密度呈负相关(rs=-0.38,P<0.05)。结论:胎盘滋养细胞浸润能力下降致绒毛血管密度减少可能是子痫前期发病的重要原因,Ang-2和HIF-1α的异常表达可能参与此发病机制。Objective: To detect the expression levels of angiogenin - 2 and hypoxia inducible factor - 1α( HIF - 1α ) in placenta tissues of patients with preeclampsia and healthy pregnant women, explore the relationship between angiogenin - 2, HIF - 1α and the pathogenesis of preeclampsia. Methods: 52 patients with preeclampsia and 20 healthy pregnant women were selected as preeclampsia group and control group, the amounts of chorial trophoblastic cells knots and density of chorial blood vessels of placenta tissues in the two groups were detected under microscope, the expression levels of angiogenin - 2 and HIF - 1α were detected by immunohistochemistry. Results : The amount of chorial trophoblastic cells knots in preeclampsia group was higher than that in control group, the density of chorial blood vessels reversed (P 〈0.05) . The expression level of angiogenin -2 in preeclampsia group was lower than that in control group (P 〈0. 05), the expression level of HIF - 1α reversed (P 〈 0. 01 ) . There was positive correlation between the expression level of HIF -1α and chorial tropho- blastic cells knots ( rs = 0. 794 P 〈 0. 05 ) , and there was negative correlation between the expression level of angiogenin - 2 and the density of chorial blood vessels (rs = - 0. 38, P 〈 0. 05) . Conclusion : The main pathogenic cause of preeclampsia maybe decreased density of chorial blood vessels induced by decreased invasion ability of trophoblastic ceils, the abnormal expression levels of angiogenin - 2 and HIF -1α may play a role in the pathogenesis of preeclampsia.
