摘　　要：目的探讨鼻中隔偏曲矫正术同时行鼻尖整形术的方法与效果。方法采用鼻小柱“V”形切口、鼻小柱人中结合部“V”形切口以及鼻侧软骨和鼻翼软骨间切口入路，显露畸形的鼻中隔软骨及侧鼻、鼻翼软骨和鼻骨结构，采用弯曲侧鼻中隔软骨切开及鼻中隔软骨完全游离等方法矫正鼻中隔畸形。利用鼻翼软骨外侧角离断或切除以提升鼻尖，缩小鼻头或利用鼻中隔软骨移植方法进行鼻尖整形，加固鼻中隔并调整鼻背对称性。适情况行“V—Y”皮瓣推进以减少鼻尖张力。结果本组共15例患者，经1～12个月的随访，均获得满意效果，无复发和并发症发生。结论本手术方法不但矫正了弯曲的鼻中隔，改善了鼻通气，同时利用鼻中隔软骨移植等方法重塑了鼻尖结构，矫正畸形的同时重塑鼻尖美容。Objective To study and evaluate the operational method and result of simultaneous correction of nasal seplum deviation and nasal tip plasty. Methods The incision of ＂V＂ shape at columellar and columellarlabial crease, of combination area between lateral nasal cartilage and greater alary cartilage was employed in all patients respectively. After exposing deviated septum, lateral nasal cartilage and nasal bone structure, nasal seplure was straightened with deviated lateral septum incising or complete septmn isolation. Nasal tip was elevated and shortened with lateral angle of alar.- cartilage interruption or excision partly and septum cartilage transplanta- tion was employed to reinforee septum and adjust the nasal back symmetry, as well as ＂V-Y＂ advancement flap lowering the tension of nasal tip. Results Deformity nose in all 15 patients were corrected satisfactorily. Normal nasal shape was obtained with no recurrence and complications being found in ＇all cases. Conclusion Not only can the procedure correct the deviated septum, improve the nasal obstruction, but also can reshape the nasal tip structure with septmn cartilage graft, which gives the double results of deformity correction and nasal tip aesthetics.
