硅胶假体置入联合鼻翼沟成形术矫正鞍鼻合并鼻尖圆钝畸形

摘　　要：目的探讨采用硅胶假体置人联合鼻翼沟成形术治疗轻、中度鞍鼻合并鼻尖圆钝畸形的疗效。方法针对鞍鼻合并鼻尖圆钝者，设计鼻翼内缘鼻小柱蝶形切口，行埋线法鼻翼沟成形术，用4旬可吸收缝线将鼻翼软骨向中线拉拢缝合固定，同时行“L”形鼻假体置入，抬高鼻背及鼻尖。结果本组53例求美者，术后鼻部美学效果明显改善，均未出现形态异常，随访6个月以上，效果良好。结论硅胶假体置入联合鼻翼沟成形术，矫正了轻、中度鞍鼻合并鼻尖圆钝畸形，改善了鼻部整体美学形态，且降低了鼻尖部皮肤张力，避免假体下滑、移位等并发症的发生，取得了良好的效果，值得临床推广应用。Objective To explore a method of treating the slight and moderate malformation of saddle nose combined with circle-blunt nasal tip by implanting silica gel prosthesis and sulcus alaris plasty. Methods The papilionaceous incision of the inner margin of the nasal wing and nasal columella was designed for the patients with saddle nose combined with circle-blunt nasal tip, then sulcus alaris plasty with buried suture was performed by drawing, suturing and fixing nasal alar cartilage to the median line with absorbable suture 4-0, and the L-shaped nasal silicone prosthesis was implanted at the same time to raise the tip and the hack of the nose. Results The nose appearance of all the 53 patients who want to pursuit beauty was greatly improved postoperatively. After more than 6 months flowing up, all the patients were satisfied with the effect of the surgery. Conclusion Under the silica gel prosthesis and sulcus alaris plasty, the overall esthetics shape of nose was improved without any complications such as the phantom sliding, shifting and revealing. This method is worth to be widely promoted and applicated in clinical.
