鼻丘气房过度气化对内镜下泪囊鼻内造口术的影响

摘　　要：目的探讨鼻丘气房过度气化在鼻内镜下泪囊鼻内造口术时对造孔的影响。方法18例溢泪患者经泪囊造影CT检查后显示鼻泪管阻塞,且鼻丘气房气化超过泪颌缝,采用经鼻内镜下鼻丘气房泪囊鼻内造口术,回顾性分析该术式及临床疗效。结果18例(20眼)术后随访1年,治愈率9 0%(18/20);显效5%(1/20);有效5%(1/20),总有效率100%。结论通过术前泪囊CT造影检查明确鼻丘气房与泪囊窝的局部解剖关系,对鼻丘气房过度发育者,可行经鼻丘气房泪囊鼻内造口术。Objective To explore surgical means of endoscopic intranasal dacryocystorhinostomy in over-developmental cells of the nasal agger.Methods 18 patients with chronic dacryocystitis,whose nasal agger cells over-passed lacrimomaxillary suture demonstrated by CT dacryocystography were performed with endoscopic intra-agger dacryocystorhinostomy.Their clinical data was analyzed retrospectively.Results Of all 20 eyes（18 cases）,18 eyes（90.0%） were cured,one eye（5.0 %） obviously improved and one eye（5.0 %） improved.The total effective rate was 100 % after one year postoperative follow-up.No complications occurred.Conclusion The anatomic relation between the nasal agger cell and fossa of lacimal sac should be identified by CT dacryocystography preoperatively.Endoscopic intra-agger dacryocystorhinostomy can be performed in those with over-developmental cells of the nasal agger.
