摘  要:目的:在不同受试者中,比较一口气法和内呼吸法测定一氧化碳弥散量(DLCO)的结果,探讨如何合理选择这2种方法。方法:应用一口气法和内呼吸法测定35名健康受试者、46例阻塞性通气功能障碍患者和22例限制性通气功能障碍患者的DLCO,每种方法重复测试2次,取平均值,2次测试间隔20min。结果:①35名健康对照者皆完成了一口气法和内呼吸法的测试,2种方法测得的DLCO间有显著相关性(相关系数r=0.989,P<0.0001),2种方法测得的结果均值间差异无统计学意义(P>0.05);2种方法测得的弥散常数(Kco)间有显著相关性(相关系数r=0.885,P<0.0001),2种方法测得结果均值间差异无统计学意义(P>0.05)。②在46例阻塞性通气功能障碍组患者中23例完成了2种方法测定,2种方法测得的DLCO间有显著相关性(相关系数r=0.997,P<0.0001),两者均值间差异无统计学意义(P>0.05);2种方法测得的Kco间有显著相关性(相关系数r=0.867,P<0.0001),两者均值间差异无统计学意义(P>0.05)。其余23例患者无法用一口气法进行测试,用内呼吸法测试则皆得到满意的结果。③在22例限制性通气功能障碍组患者中21例完成了2种方法测试,2种方法测得的DLCO间有显著相关性(相关系数r=0.996,P<0.0001),两者均值间差异无统计学意义(P>0.05);2种方法测得的Kco间有显著相关性(相关系数r=0.863,P<0.0001),两者均值的差异无统计学意义(P>0.05)。④一口气法测试时,3组受试者成功率不同(P<0.05),以健康受试者成功率最高;应用内呼吸法测试时,3组受试者的成功率不同(P<0.05),以阻塞性通气功能障碍组患者的检测成功率为最高。结论:①不同组别间,一口气法与内呼吸法测定DLCO的结果一致性好,差异无统计学意义;②内呼吸法的适应证较一口气法更广泛;③采用一口气法测试时,健康受试者的检测成功率高,而采用内呼吸法测试,则阻塞性通气功能障碍患者的检测成功率高Objective To compare the results of DLCO measured by single breath method and intrabreath method in various kinds of pulmonary diseases and to decide how to choose a method. Methods Forty-six patients with obstructive pulmonary disease and 22 patients with restrictive pulmonary disease were enrolled, with 35 healthy subjects served as controls. DLCO was measured by single breath method and intrabreath method. Each test was performed twice with an interval of twenty minutes. Results ① All 35 healthy subjects completed the test with the two methods. There was no significant difference between the results of intrabreath method (DLCO IB) and single breath method (DLCO SB). Excellent correlation between DLCO IB and DLCO SB was noted (r=0.989, P〈0.0001). There was also an excellent correlation between diffusion constant Kco IB and Kco SB (r=0.885, P〈0.0001)②Among 46 patients with obstructive pulmonary diseases, only 23 patients completed the test with two methods. There was no significant difference between the results of intrabreath method (DLCO IB) and single breath method (DLCO SB). Excellent correlation between DLCO IB and DLCO SB (r=0.997, P〈0.0001) was noted. There was also an excellent correlation between Keo IB and Keo SB (r=0.867, P〈0.0001). Other 23 obstructive puhnonary patients could not be measured with single breath method but completed the test with intrabreath method. ③ Of 22 patients with restrictive pulmonary diseases, 21 patients completed the test with the two methods. There was no significant difference between the results of the two methods. The correlation between DLCO SB and DLCO IB was excellant (r=0.996, P〈0.0001). There was also an excellent correlation between Kco IB and Kco SB (r= 0.863, P〈0.0001).④When DLCO was measured by single breath method, the success rate was the highest in healthy subjects (P〈0.05), while the intrabreath method had the highest success rate in obstructive pulmonary disease group (P〈 0.05).
