摘 要:目的利用基因芯片技术分析人乳头瘤病毒(HPV)在天津地区外阴尖锐湿疣(CA)患者中的基因亚型,探讨高危型HPV感染引发的CA临床和病理特征。方法采用基因芯片方法检测118例CA患者中HPV基因亚型(包括5种低危型和10种高危型)。结果118例CA标本中检出HPV阳性117例,阳性率为99.15%。共检测出7种亚型,其中低危型(HPV6,11,6+11)感染108例,占92.31%;高危型(HPV16,33)感染3例,占2.56%;高/低危型(18+6,31+6,31+11,33+11,35+11)混合感染6例,占5.13%。6例女性高危型及高/低危混合型感染患者中,5例合并一种或二种妇科炎性疾患。高危型HPV感染者组织病理像可有细胞异型性改变,细胞核大而深染,形态不规则,可见核分裂像。结论基因芯片技术适用于CA患者的HPV亚型检测,6型是本地区的主要致病亚型;发现高危型及高低危混合HPV感染的一些临床、病理特征,但尚需大样本观察。Objective Gene chip was employed to analyze the main gene subtypes of human papillomavirus( HPV ) of condyloma acuminatum (CA) patients in Tianjin. The clinical and pathological features of CA caused by highrisk type HPV infection were discussed. Methods Gene chip was used to test the gene subtypes of HPV in 118 CA patients, including 5 low-risk types and 10 high-risk types. Results Among the 118 CA samples tested, 117 showed positive, and the positive rate was 99.15%. The test result revealed 7 HPV gene subtypes including 108 low-risk-infected cases (HPV6, 11, 11 + 6),occupying 92.31%; 3 high-risk-infected cases ( HPV16, 33), occupying 2.56% ; and 6 mixed-infected cases (18 +6, 31 +6, 31 + 11, 33 + 11, 35 + 11), occupying 5.13%. Among the high-risk-infected and mixed-infected female patients, five out of six had 1 or 2 kinds of gynecological inflammation. The cellular atypical changes existed on the tissue of the high-risk HPV infectors. Compared with that of the normal, the nucleuses were bigger, darker staining and more irregular; the karyokinesis could be easily seen. Conclusion The gene chip technology could be applied to the HPV subtype test for CA patients. HPV 6 is a major subtype in the local region. Some clinical and pathological features of high-risk and mixed type HPV infection are found, but the larger samples should be investigated.
