体外受精-胚胎移植周期中血清性激素水平对胚胎着床的影响

摘　　要：目的探讨IVF周期中血清雌、孕激素水平及其比值与胚胎着床关系。方法行IVF-ET治疗38例患者共38个周期(其中妊娠组17例、未孕组21例),用化学发光法测定HCG日、胚胎移植日、胚胎种植窗血清孕酮、雌二醇水平;统计学方法t检验。结果两组黄体支持黄体酮剂量差异无统计学意义(P>0.05),妊娠组与未孕组HCG日、胚胎移植日、种植窗血清雌、孕激素水平升降变化趋势一致,但妊娠组以上三个时期雌二醇水平高于未孕组,但差异无统计学意义(P>0.05),种植窗孕酮高于未孕组(P<0.05);妊娠组种植窗与移植日E2/P值明显低于未孕组,差异无统计学意义(P>0.05)。结论IVF-ET周期患者自身黄体功能影响妊娠结局,血清E2/P比值过高可能影响胚胎着床。Objective To study the relationship between the level of serum Estradiol （E2）, Progesterone （P） and E2/P during luteal phase and embryo implantation in IVF cycle. Methods 38 patients involved in this study were divided into two groups： pregnancy group （n = 17） and non - pregnancy group （n = 21）. Serum E2 and P levels were determined on the HCG administration day, embryo transfer day and 3 days after embryo transfer （embryo implantation window）. Results The same progesterone support was found in the two groups. Serum P and F4 levels were at the same variable tendency on HCG administration day, embryo transfer day and embryo implantation window in the two groups. Serum P and F-4 levels were higher in pregnancy group than those in non- pregnancy group on embryo transfer day and embryo implantation windown, but no significant differences were found. Serum P levels in pregnancy group were higher than those in the non - pregnancy group on embryo implantation window （P〈 0. 05） ; the ratio of E2/P on the embryo transfer day and embryo implantation window were lower in the pregnancy group than the nonpregnancy group, but no significant differences were found in the two groups （P〉0.05）. Conclusion Luteal function of the patient itself may affected the clinical pregnancy rate in IVF cycle, higher ratio of E2/P in luteal phase may be damaged to embryo implantation.
