全景片在下颌第三阻生磨牙拔除中预测下齿槽神经损伤的应用

摘　　要：目的 评价全景片在下颌第三磨牙拔除中预测下齿槽神经损伤的应用价值。方法对149名患者总计220颗下颌第三磨牙于拔除前拍摄全景片并进行图像分析。在133颗牙的全景片上观察到牙根与下齿槽神经管紧密接触的指征。结果220颗第三磨牙中，有7颗牙在拔除后出现了术后神经损伤症状。得出阳性预测值和阴性预测值分别为0．03和0．97，灵敏度为57％，特异度为39％。结论在第三磨牙的拔除中，全景片图像用于预测下齿槽神经损伤诊断价值较低。Objective To assess the sensitivity and specificity of panoramic radiogaphy in predicting nerve lesions in third molar surgery. Methods Panoramic radiographs of 220 third molars in the sample of 149 patients were assessed prior to surgical removal. The radiological sign of the relationship between the mandibular canal and third molars was recorded in 133 cases. Results Of the 220 cases, 7 presented postsurgical sensory complications. The positive and negative predictive values were 0.03 and 0.97 respectively, while the sensitivity and specificity were 57% and 39%. Conclusion Panoramic radiograph does not provide the reliable images required for predicting nerve lesions in third molar surgery.
