摘 要:目的 评价超声心动图、64排螺旋CT(MSCT)及电影心血管成像(CAG)对于紫绀型先天性心脏病肺血管成像的临床应用价值。方法回顾42例紫绀型先天性心脏病患者的超声心动图、MSCT、CAG影像学资料,并和手术结果对比,进行统计学分析。其中男29例,女13例,年龄11d~28岁,平均7.26岁。结果本组42例患者经手术证实肺血管共有109处畸形。超声心动图正确诊断93处畸形,符合率85.3%;漏诊误诊16处;MSCT确诊102处畸形,符合率93.6%;漏诊误诊7处畸形;CAG确诊106处畸形,符合率97.2%;漏诊误诊3处畸形。超声心动图和MSCT、CAG对比诊断符合率均有显著性差异。MSCT和CAG对比诊断符合率无显著性差异。结论MSCT对紫绀型先天性心脏病肺血管成像具有较高的临床应用价值。Objective To evaluate the accuracy of echocardiography, 64-slice spiral CT (64-slice SCT) and cineangiocardiograms (CAG) in detecting the pulmonary vessels in cyanotic heart disease. Methods 42 patients with cyanotic heart disease(29 men and 13 women;the age ranged from 11 days to 28 years, mean 7.26 years) underwent echocardiography, 64-slice SCT and CAG. The imaging data were analyzed statistically and compared with surgical findings. Results There were 109 deformities confirmed by surgery. The accurate rates of echocardiography, 64-slice SCT and CAG in the evaluation of the pulmonary vascular deformities were 85.3 % (93/109 ), 93.6% (102/109 ) and 97.2% (106/109), respectively. The differences among t]aem were significant, the accurate rates of 64-slice SCT and CAG were higher than that of echocardiography, but there was no differences between 64-slice SCT and CAG. Conclusion 64-slice SCT is of high accuracy in the evaluaton of the pulmonary vessels in cyanotic heart disease.
