两种方法治疗卵巢子宫内膜异位囊肿疗效比较

摘　　要：目的：比较腹腔镜与开腹保守性手术治疗卵巢子宫内膜异位囊肿的疗效。方法：对我院2002年12月-2006年11月实施的卵巢子宫内膜异位囊肿腹腔镜保守性手术54例和开腹保守性手术71例临床资料进行回顾性分析。结果：腹腔镜组治疗痛经总有效率84.6%（22/26）,性交痛总有效率88.2%（15/17）,慢性盆腔痛总有效率89.2%（33/37）,复发率11.5%（6/52）;开腹组治疗痛经总有效率88.2%（30/34）,性交痛总有效率87.5%（21/24）,慢性盆腔痛总有效率89.8%（44/49）,复发率13.2%（9/68）;两组四项比较差异均无统计学意义（P〈0.05）;对比两组对卵巢功能影响率,半年内差异有统计学意义（P〈0.05）,两年内两组总妊娠率差异有统计学意义（P〈0.05）。结论：对保留生育功能的卵巢子宫内膜异位囊肿患者,腹腔镜手术是的安全、有效的手术方法。Objective To compare the effect of laparoscopic and open conservative surgery in the treatment of ovarian endometrial cysts.Method The clinical data of 125 cases（54 cases of laparoscopic group,71 cases of transabdominal group）,admitted to our department between December 2002 and November 2006 were retrospectively analyzed.Results When laparoscopic group was compared with transabdominal group,it had no statistical significance in the effective rate of dysmenorrhea,algopareunia,chronic pelvic pain and recurrence rate （ P 〉 0.05 ）. However, laparoscopic group was better than transabdominal group in the effect of ovarian function in half a year after operation（ P 〈 0. 05 ） , and in pregnancy rate in two years after operation（ P 〈 0. 05 ）. Conclusion Laparoscopic conservative surgery is safe and effective for the patients with endometrial cysts of ovary.
