Comparison of Postoperative Complications and Patients' Comfortableness in Percutaneous Coronary Interventional Treatment by Two Approaches

摘  要:目的对经桡动脉途径和经股动脉途径行冠状动脉介入治疗(percutaneous coronary intervention,PCI)的术后并发症及患者舒适度进行对照研究,观察两者差异情况。方法选择冠心病患者338例,随机分为桡动脉组153例和股动脉组185例,比较两组患者术后并发症和舒适度的情况。结果患者局部血肿、伤口出血、假性动脉瘤、迷走神经反射发生率及患者舒适度桡动脉组明显低于股动脉组(P<0.01、P<0.05、P<0.01及P<0.01);动脉痉挛或闭塞发生率桡动脉组明显高于股动脉组(P<0.01),差异有统计学意义。结论桡动脉途径较股动脉途径PCI术后不易发生伤口出血、血肿、假性动脉瘤及迷走神经反射,且患者更加舒适,但应注意防止桡动脉痉挛的发生。Objective To conduce a control study of postoperative complications and patients' comfortableness in percutaneous coronary intervention(PCI) by radial artery approaches and femoral artery approaches so as to observe the differences between the two groups. Methods A total of 338 patients with coronary artery disease were randomly divided into two groups: radial artery group (n= 153)and femoral artery group (n= 185). A comparison was conducted in the postoperative complications and comfortableness between the patients in the two groups. Results The incidences of Local hematomas, wound bleeding, pseudoaneurysm, vagus nerve reflex and comfortableness in radial artery group were significantly lower than those of femoral artery group (P〈 0.01, P〈0. 05, P〈0.01 and P〈0. 05). The incidence of artery spasm or occlusion in radial artery group was significantly higher than that in femoral artery group (P〈0. 01). There were signifi- cant statistical differences between them. Conclusion The occurrences of wound bleeding,hematoma,pseudoaneurysm,vagus nerve reflex were less and comforts were more in the patients with PCI by radial artery approach, but radial artery spasm after PCI deserved full awareness.
