复合激光和红光穴位照射治疗膝骨关节炎的观察及护理干预

摘　　要：目的观察复合激光和发光二极管红光穴位照射治疗膝骨关节炎患者的临床疗效,并总结护理对策。方法将2008年4月至2009年3月在骨科、针灸科门诊就诊的膝骨关节炎患者分为A组(复合激光红光穴位照射)、B组(发光二极管穴位照射)和常规治疗组,接受为期6周的治疗,共治疗14次。符合纳入标准并完成治疗者共48例,每组16例。采用西安大略和麦克马斯特大学骨关节炎调查量表(WOMAC VA3.1)进行疗效评价。结果A、B组患者在治疗2周、6周及治疗结束4周后,WOMAC疼痛、僵硬、功能得分均降低(P<0.01),两组与常规治疗组相比差异有统计学意义(P<0.05或0.01);复合激光组患者对疗效的自评优于发光二极管组(P<0.05或0.01)。结论复合激光和发光二极管红光穴位照射对膝骨关节炎患者的膝关节疼痛及功能障碍有显著疗效,患者对前者的自评疗效更佳,而针对性的护理干预对保证疗效有重要意义。Objective To observe the clinical and nursing effects of the combined laser or RLED Phototherapy in patients with knee osteoarthritis （OA） and summarize the nursing strategy. Methods The patients with knee OA in orthopaedics department and acupuncture department from Apr. 2008 to Mar. 2009 were randomized into three groups： the combined laser group（group A） ,RLED group （group B） and regular treat group withl4 times treatment for 6 weeks. Forty- eight patients met inclusion criteria and completed their treatments with 16 patients in each group. The main outcomes were evaluated with Western Ontario and McMaster Universities＇ Osteoarthritis Index （WOMAC VA3.1） scores. Result The WOMAC scores on pain, stiffness and function, in group A and group B reduced significantly （P 〈0. 01） and statistical significant differences were observed between the two treatment groups and the regular group after 2 and 6 weeks treatment and 4 weeks after treatment（P〈0.05 or 0.01）. The patients＇ assessment showed the therapeutic effects of the combined laser group were superior to those of the RLED group （P〈0.05 or 0. 01）. Conclusion The treatments of combined laser and the RLED have special effects for the patients with knee OA and dysfunctions. The self-assessments of patients treated with the combined laser were superior to those treated with RLED. The targeted nursing interventions can ensure the effectiveness of treatment.
