下肢手术气囊止血带最适充气压力的探讨

摘　　要：目的探讨下肢手术中使用气囊止血带的合适充气压力。方法对10例自愿者采用彩色多普勒诊断仪测量下肢动脉血阻断所需的压力,确定适宜的止血带充气压力;对38例择期下肢手术患者,根据自愿者测得的止血带充气压力加120 mmHg作为患肢手术止血带基础充气压力,再根据患者下肢周径适当增减,于止血带充气前(T0)、充气后即刻(T1)、使用止血带30 min(T2)、松开止血带后(T3)分别测定记录各时间点血压、脉搏、呼吸的变化。结果38例患者术中止血带使用达优率为100%,止血效果满意。各时段的血压、脉搏、呼吸均有不同程度的变化,但差异无统计学意义(均P>0.05);结论在下肢手术中,采用适宜的气囊止血带充气压力既不影响止血效果,又能减轻止血带的不良反应。Objective To determine the right pressure of tourniquet during operation on lower extremity. Methods The suitable pressure of tourniquet was determined in 10 volunteers by block pressure test in the arterial blood of lower extremity using color Doppler. The tourniquet blood for lower extremity operation was a combination of 120 mmHg and the pressure measured in 38 patients undergoing lower extremity operations. The blood pressure pulse and breath were recorded before the inflation of tourniquet（T0） ,immediately after the inflation（T1 ）, 30 rain after the inflation of tourniquet（T2） and after removal of tourniquet（T3） in the patients. Results The excellent hemostastic effect was achieved in 100% of the patients. Changes were observed in the blood pressure, pulse and respiration with significant differences（P〉0.05）. Conclusion The suitable pressure does not affect the hematischesis effects and can reduce the adverse reactions for the patients.
