静脉营养对AECOPD机械通气患者肺功能的影响

摘　　要：目的探讨静脉营养支持对合并营养不良的AECOPD行机械通气治疗患者肺功能的影响。方法43例合并营养不良的COPD急性加重期行机械通气的患者随机分成两组,即静脉营养组和对照组,观察两周前后肺功能的变化。结果(1)人体测量及生化检测:PN组WT、IBW%、TSF、MAMC、PALB明显增加(P<0.05),对照组无明显增加(P>0.05)。(2)肺功能和血气检查:PN组的FVC、FEV1/FVC有明显改善(P<0.05),PN组的PaO2、PaCO2有显著改善(P<0.01);对照组上述指标无显著变化。结论静脉营养支持可以改善行机械通气的COPD急性期患者的营养状况,进而改善其肺功能。Objective To explore the effect of parenteral nutrition in COPD patients with mechanical ventilation. Methods 43 patients with COPD were randomly divided into two groups： parenteral nutrition （PN） group and control group. Pulmonary function was measured two weeks after PN support. Results （1） Anthropometric measurement and biochemical： IBW%, TSF, MAMC and blood serum albumin in PN group significantly increased （ P 〈0.05 ）. （2） Measurements of pulmonary function and arterial blood gas ： FVC, FEVl/FVC, PaO2 in the PN group significant improved （ P 〈 0.05 and P 〈 0.01）, more than in the control group. Conclusion Paren- teral nutrition can improve nutritional status and pulmonary function in COPD patients undcrgoing mechanical ventilation.
