冠状动脉旁路移植术后纵隔、心包引流管放置时间与疼痛的关系

摘　　要：目的分析冠脉动脉旁路移植术（CABG）后纵隔、心包引流管（心纵引流管）放置的时间与术后疼痛之间的关系。方法选取2002—2003年125例CABG术后患者，均为正中劈开胸骨直视下进行，随机分为早拔管组61例和晚拔管组64例。晚拔管组引流放置时间较长，为在术后第2天或者第3天心纵引流完全消失后拔除心纵引流管。早拔管组引流放置时间较短，拔管方案为术后6h内心脏引流〈200ml即于术后第1天拔除心脏引流管。比较2组患者的纵隔感染情况及疼痛发生情况。结果术后早拔管组患者主诉疼痛发生率稍低于晚拔管组。且2组病例均未出现需要拔管后再次置入心纵引流或进行心包或胸腔穿刺引流，2组术后第2天和第3天的疼痛发生率比较有显著差异。结论本研究结果提示CABG术后早拔除心纵引流可以减少患者疼痛的发生。因此，为减少患者疼痛发生，在适宜条件下，术后早拔除心脏引流管是可取的。Objective To know the relationship between indwelling time of drainage tube of mediastinum and pericardium and pain of patients with coronary arteries bypass grafting surgery. Methods Divided 125 patients with CABG into the early drop group（61 cases） and the later drop group（64 cases）randomly. Different indicator of drop tube was used in the different group. Compared the infection condition of mediastinum and the postoperative pains between the two groups. Results There was no re-indwelling cases in beth groups after drop the drainage tube. The degree of postoperative pain in the early drop group was better than that in the later drop group. The difference of indicence rate of pain at the 2nd and the 3rd day after the operation between the two groups was significantly. Conclusions Drop the drainage tube after the CABG as early as possible can avoid part of postoperative pain, which should be carried out at proper time points.
