PCT、IL-6、CRP在早期诊断极低出生体重儿败血症中的意义

摘　　要：[目的]评价降钙素原(PCT)、白细胞介素-6(IL-6)及C-反应蛋白(CRP)在极低出生体重儿败血症早期诊断的价值。[方法]对我院新生儿病房收治的102例极低出生体重儿按败血症诊断标准分感染组(确定诊断和临床诊断)共55例和无感染组对照47例,于入院后、d3、d7检测血清PCT、IL-6及CRP。[结果]败血症患儿入院时PCT、IL-6及CRP均较对照组升高,抗生素治疗后迅速下降,PCT诊断敏感性最高,达92.7%,其次为IL-6及CRP,分别为85.5%和70.9%,PCT诊断的特异性也最高,为95.7%,CRP为87.2%,而IL-6最低为83%。[结论]PCT对极低出生体重儿败血症早期诊断有较高敏感性,可作为早期诊断的首选指标。[Objective] To investigate the clinical significance of procalcitonin（PCT） interleukin-6（IL-6）,and Creactivep rotein（CRP） in early diagnosis of low birth weight infant sepsis.[Methods] 102 low birth weight infants in our neonatology ward were divided into sepsis group（n=55） and non-infection control group（n=47） according to the diagnostic criteria of sepsis.Serum PCT,IL-6,and CRP were measured at the first,third,and seventh day after admission,respectively.[Results] The levels of PCT,IL-6,and CRP were significantly higher in sepsis group than that in control group and decreased with the time.The sensitivity of diagnosis of sepsis at the first day after admission was 92.7% for PCT,85.5% for IL-6 and 70.9% for CRP,respectively.The specificity of diagnosis of sepsis at the first day after admission was 95.7% for PCT,83% for IL-6 and 85% for CRP,respectively.[Conclusion] PCT has a highest sensitivity and specificity than IL-6 and CRP for early diagnosing low birth weight infant sepsis.
