摘  要:目的了解我院连续3年临床尿路感染标本中病原菌分布及对常用抗菌药物的耐药现状。方法采用常规方法分离鉴定细菌;抗菌药物敏感性试验应用K-B纸片扩散法,按CLSI规定的标准进行;试验结果以WHONET5.4软件进行统计分析。结果3年内从尿路感染标本中共分离获得1269株病原菌,其中革兰阳性球菌389株(占30.65%),革兰阴性杆菌731株(占57.60%),假丝酵母菌137株(占10.80%),前5位病原菌依次为大肠杆菌、屎肠球菌、粪肠球菌、表皮葡萄球菌、肺炎克雷伯菌;大肠杆菌产ESBL酶为40.66%(238/516),耐甲氧西林表皮葡萄球菌占82.42%(75/91);大肠杆菌对碳青霉烯类抗菌药物全敏感,肠球菌和表皮葡萄球菌对万古霉素、替考拉宁和利奈唑胺均敏感,未检出耐药菌株。结论临床尿路感染病原菌中以革兰阴性杆菌为主,最常见的细菌为大肠杆菌和肠球菌;碳青霉烯类抗菌药物对肠杆菌科细菌的敏感性最高,万古霉素、替考拉宁和利奈唑胺对革兰阳性球菌均具有良好的敏感性。Objective To understand distribution and drug resistance of pathogens from clinical urine tract infection samples in three years in our hospital. Methods Asolates were cultured and identified by routine method. Antimicrobiotics susceptibility test used K - B paper disk diffution method in accrodance to CLSI standard. Analysis and statistics of results used WHONETS. 4 soft. Results 1269 strain pathogens were isolated from urine tract infection samples, in which 398 strains were gram positive cocci(30.65% ) ,731 strains wrer gram negative bacillus(57.6% )and 137 strains were candida species (10.8%). The range from one to five was E. Coli, enterococcus faecium,Entercoccus faecalis, staphylococcus epidermidis, klebsiella pheumoniae. Producing ESBLs rate of E. coli was 40.66% (238/ 516). Methicillin resitance staphylococcus epidermidis was 82.42% (75/91). E. Coli was all sensitive to carbapenems. Enterococci and staphylococci epidimidis against vancomycin, teicoplanin and linezolid were all sensitive. Conclusion Gram negative bacillus is main pathogens in clinical urine tract infection. The important baeteria are E. Coli and Enterococci. Carbapenems against enterobacteriaecae and vaneomycin, teicoplanin linezolid against gram positive cocci are very good sensitivity.
