摘  要:目的通过比较1.44μm与1.06μm脉冲Nd∶YAG激光对犬膀胱组织作用的异同点,评价1.44μm脉冲Nd∶YAG激光的组织效应特点、临床应用的可能性与特异性。方法采用脉宽640μs,频率5Hz的1.44μm与1.06μm脉冲Nd∶YAG激光,按单脉冲能量0(对照)、200、500和800mJ与每次照射脉冲数1、5、10、20、40和80进行分组。两个波长、由相同单脉冲能量指标构成的12个剂量组和一个对照组在一个膀胱后壁不同部位进行活体照射,全部实验完成一遍共3只犬,重复5遍。取照射处膀胱壁制作组织切片,显微镜观察,计算机成像与测量,获取气化宽度、气化深度、凝固宽度与凝固深度数据,进行数据处理与分析。结果1.44μm脉冲Nd∶YAG激光对膀胱组织的气化能力远大于1.06μm脉冲Nd∶YAG激光,同时其凝固作用足以产生良好的止血效果。结论1.44μm脉冲Nd∶YAG激光对犬膀胱黏膜有较强气化与凝固止血作用,作为手术刀应用于膀胱手术,其切割效果远高于1.06μm脉冲Nd∶YAG激光,并能在切割手术同时表现出良好的止血功能。Objective To evaluate the characteristic of histologic effects and the feasibility and superiority of 1.44μm Nd : YAG laser by comparing the acute effects of 1.44 μm with 1.06 μm Nd : YAG laser radiation on bladder wall in dogs. Methods A pulse Nd : YAG laser which could produce irradiation at either 1.44 μm or 1.06 μm wavelength with a pulse duration of 640 μs and a frequency of 5 Hz was used in this experiment to fifteen beagle dogs. The bladder wall in each dog was irradiated with different pulse energy at 200,500 and 800 mJ ( 1,5,10,20,40 and 80 pulses/spot) producing 36 different lesioning data points and 1 norreal eontrol(0mJ and 0 pulse ) points in which coronal plane sections were made. By means of light miereseope and taking photomicrograph, the width and depth of the histological zones of the lesions including photoevaporated zones and coagulation neeresis zones were measured. The histological characteristics of 1.44 μm laser were discussed. Results The bladder wall could be effectively photoevaporated as well as coagulated by 1.44 μm Nd:YAG laser. The photoevaparatlve ability of 1.44 μm Nd : YAG laser was higher than 1.06 μm Nd : YAG laser, meanwhile its hemestatie effects was good enough in the operation.Conclusions 1.44 μm Nd : YAG laser is suitable for bladder surgery, with a higher ablation efficiency than the 1.06 μm laser while maintaining effective hemostasis.
