噻托溴胺粉雾剂对稳定期不同型慢性阻塞性肺疾病的疗效观察

摘　　要：目的：观察噻托溴胺粉雾剂对稳定期不同型慢性阻塞性肺疾病（COPD）患者的治疗效果。方法：将选取的60例稳定期的COPD的患者分为两组,A组30例,胸片提示为肺气肿型,B组30例,胸片提示为肺纤维化型,两组均给予噻托溴胺粉雾剂吸入治疗。于观察首日、第8周、第12周,检测1 s用力呼气量（FEV1）、FEV1占预计值的百分比[FEV1（%）],用力呼气量（FVC）、FEV1/FVC、深吸气量（IC）等指标。随访3～6个月急性加重的人次、住院人次。结果：A组FEV1在第8周与B组比较显著升高（P〈0.05）,第12周差异无显著性（P〉0.05）;FEV1（%）在第8周、12周与B组比较差异无显著性（P〉0.05）;FVC在第8周、12周较B组均有显著升高（P〈0.05）;FEV1/FVC在第8周、12周与B组比较,差异无显著性（P〉0.05）;IC在第8周、12周较B组显著升高（P〈0.05）;随访3～6个月A组急性加重人次、住院人次与B组比较明显减少,差异有显著性（P〈0.05）。结论：噻托溴胺粉雾剂对气肿型COPD患者肺功能的改善作用优于肺纤维化型,明显减少气肿型COPD患者急性加重。Objective： To observe the clinical effect of tiotropium in the treatment of COPD patient at stable stage.Methods： 60 patients with COPD were rFEV1andomly divided into 2 groups,30 patients in A group were indicated gas swelling type in chest x-ray,30 patients in B group were indicated lung fiberalition type.At each of visits dags 1,57（about 8 weaks）and 85（about 12 weaks）,forced expiratory volume in one second（FEV1）percentage of FEV1 and predict volume [FEV1（%）]、forced expiratory volume（FVC） .FEV1/FVC and IC were measured. To review the person - time of becoming serious acutely and in - patient in 3 - 6 months. Results： Compared with B group, FEV1 in A group increased singnificantly in weak 8 （P 〈 0.05 ）, but there was no statistical signieance between than in weak 12 （ P 〉 0.05 ） ; FEV1 （ % ） in weak 8 and 12, there was also no statistical significance between two groups （P 〉 O. 05） ; FVC increased significantly in 8 and 12 （ P 〈 0.05, respectively）. There was no statisticd significantly in FEV1/FVC between two groups , IC increased significantly in weak 8 and 12 （P 〈 0.05, respectively）. Conclustion：The treatment of tiotropium to gas swelling type COPD patients is better than the lung fiberation type . It can release the person - time of the COPD patient which becoming serious acutely.
