静滴丙种球蛋白治疗重症毛细支气管炎疗效观察

摘　　要：目的:观察静滴丙种球蛋白(IVIG)治疗重症毛细支气管炎的临床疗效和安全性。方法:将重症毛细支气管炎患儿64例随机分为治疗组33例和对照组31例。两组均给予抗感染、抗病毒、止咳、化痰、平喘、吸氧,应用肾上腺糖皮质激素等综合治疗。治疗组在此基础上静滴IVIG400mg/(kg·d),1次/d,连用3d,观察两组疗效及不良反应,并测定治疗前后免疫球蛋白水平。结果:两组在咳嗽、喘憋、呼吸困难和肺部喘鸣音消失时间及住院时间比较,差异有统计学意义,P值均<0.01。两组总有效率比较,差异有统计学意义,P<0.01。应用IVIG后,IgG水平升高,P<0.01,不良反应两组间比较,差异无统计学意义,P>0.05。结论:IVIG治疗婴幼儿重症毛细支气管炎可缩短病程,疗效确切,安全性高,未见不良反应,可在临床推广应用。Objective：To observe the clinical efficacy and safety about intravenous gamma globulin （IVIG）treatment of severe bronchiolitis. Methods：64 children with severe bronchiolitis were divided into treatment group 33 cases and 31 cases of the control group randomly. Both groups were given anti-infective, anti-virus, cough, phlegm,asthma, oxygen, application of adrenal glucocortieoid comprehensive treatment. Based on it, the IVIG 400 mg/（kg·d） was administered in the treatment group,once a day,three days used in conjunction.To observe the efficacy and adverse reactions,and measured the level of immunoglobulin. Results：The two groups in cough,asthma,breathing difficulties and wheezing lungs sound disappear time and hospitalization was more significant difference （P〈0.01）. The total efficiency of the two groups were significantly different （P〈0.01）. Alter the application of gamma globulin, the level of IgG were significantly higher（P〈0.01）, adverse reactions between the two groups were not significant difference （P〉0.05）. Conclusion：IVIG treatment with severe bronchiolitis may shorten the course, the exact efficacy,safety,no adverse reactions, can be applied in clinical.
