围手术期预存、回收、回输自体血在髋、膝关节置换术中的联合应用

摘　　要：目的探讨预存式自体血和术中、术后回收、回输自体血在髋、膝关节置换术中的联合应用效果。方法收集2000年6月至2008年12月,我院治疗84例病人107个髋、膝关节置换手术(其中单髋关节置换手术12例)。结果84例病人术中及术后回输预存式自体血400～800mL,平均560mL、回收式自体血200～500mL,平均320mL,术后通过CBC-Ⅱ回收自体血400～900mL,平均650mL。结论在行髋、膝关节置换中,联合采用预存自体血、术中血液回输及术后CBC-Ⅱ回收自体血进行回输技术。可提高手术安全性,减少围手术期同种异体血的输血量。Objective To explore the combined predeposit and salvage autotransfusion in knee arthroplasty. Methods 84 cases of patients hospitalized for treatment of 107 knee joints replacement surgery from June 2000 to December 2008 were studied. Results 400-800mL of predeposit and 200-800 ml of salvage blood were autotransfused in 84 cases of operative group,combined with an average of 530 ml and 450mL predeposit and salvage blood respectively,and allogeneic blood were transfused on average 340mi（0-800mL）.An average of 880mL （400-1400mL） allogeneic blood were transfused in the control group at the same period. The former significantly less than the latter （P〈0.001）on allogeneic blood transfusion. Conclusion The combined application of predeposit and salvage autotransfusion in knee arthroplasty improved the surgical safety,reduced the allogeneic blood transfusion.
