颅内压增高时岩下窦改变的CT初步研究

摘  要:目的观察颅内压增高时岩下窦(inferior petrosal sinus,IPS)的多层螺旋CT(multi-slice spiral CT,MSCT)影像学改变。方法临床收集8例颅内压增高患者和120例正常人IPS的CT资料,通过连续断面观察及多种后处理技术,对IPS分段测量后对比两组IPS的影像学变化。结果颅内压增高组:IPS起始部宽度为(9.56±2.31)mm,深度为(2.39±0.55)mm;垂直段直径为(1.76±0.60)mm;出颅处直径为(2.28±0.44)mm;汇入颈内静脉(internal jugular vein,IJV)处的直径为(2.46±0.59)mm。与正常组比较显示:IPS起始部的宽度、出颅处直径大于正常组,IPS起始部的深度、垂直段直径、汇入IJV处的直径均小于正常组。结论IPS在颅内压增高时存在一定程度的影像学改变,MSCT是评价IPS影像学变化的准确影像学检查技术。Objective To observe the changes of inferior petrosal sinus (IPS) in intracramal hypertension using multi-slice spiral CT (MSCT) , and provide image data for clinical diagnosis and treatment. Methods CT data about IPS features were collected from 8 patients with intracranial hypertension (from November 2008 to March 2009) and 120 normal subjects. The morphology of IPS was observed and measured between the 2 groups through serial thin sectional images and postprocessing techniques. Results The IPS group had a higher intracranial pressure, the width of initial part of IPS was (9.56 ± 2.31 ) mm and the depth was (2.39 ± O. 55) mm; the diameter of vertical part of IPS was ( 1.76 ±0. 60) mm; the diameter of IPS at the point leaving the cranium was (2.28 ±0.44) mm and the diameter of IPS joining internal jugular vein (IJV) was (2.46 ±0.59 ) ram. The width of initial part of IPS and the diameter of IPS at the point leaving the cranium were greater than those of the normal group. The depth of initial part of IPS, the diameter of vertical part and the diameter of IPS joining IJV were lesser than those of the normal group. Conclusion There are some morphologic changes of IPS when intracranial pressure is increased. MSCT is an accurate imaging technique to evaluate morphological features of IPS.
