乳腺肿块周围结构钼靶X线表现在定性诊断中的意义

摘　　要：目的探讨乳腺肿块周围结构表现在定性诊断中的意义,提高钼靶X线对乳腺病变的诊断水平。资料与方法搜集2005年至2007年经手术和病理证实的乳腺肿块87个,对肿块周围乳腺结构异常的有无及异常的类型、特点及在各种病变中的检出情况作回顾性分析。结果本组乳腺结构粘连扭曲、病理结构、结构正常三种病灶周围结构类型在良、恶性病变中的检出率差异有显著性统计学意义(P<0.01);结构紊乱、结构模糊和结构推移在良、恶性病变中的检出率差异无显著性统计学意义(P>0.01)。结构粘连扭曲和病理结构在乳腺恶性病变的检出率最高,其敏感性和阳性预测值分别为53.7%、88.0%和56.1%、88.5%;病灶周围结构正常者在良性病变多见(60.9%)。结论乳腺肿块周围结构的影像学表现对肿块性质的判断具有重要意义,应重视对此间接征象的观察。病灶周围结构的扭曲粘连及异常结构的检出高度提示恶性可能,病灶周围结构正常则倾向支持良性病变;伴随结构紊乱、结构模糊或结构推移的病灶,需充分结合临床表现及其他影像学表现加以鉴别。Objective To explore the value of of structure manifestation near the breast masses in determine the nature and to improve the X-ray diagnostic level in breast disease. Materials and Methods 87 breast tumors confirmed by operations and Pathology during 2005 - 2007 were collected. The types, characteristic, and frequency of different kinds of structure near the breast tumors were Retropectly analyzed. Results Three types of structures-distortion, pathological and normal structure had obviously different betweet benign and malignant tumors （ P 〈 0.01 ）. There were no obvious difference of the other three types of structures-disorder, fuzzy and constricted were showed between benign and malignant tumors. Distortion and pathological structures were mostly found near the malignant lessions, while normal structures were mostly found near the benign lessions（60.9% ）. Conclusion We should pay more attendtion to the structure near the breast lesions,because it has important value in differentiating the tumors. The discovery of distortion and pathological structures near the tumors point out high probably of maliganant. Nornmal structure is the sign of benign disease. The lesions accompanied with disorder,fuzzy and constricted structures were difficult to decide the nature only if comprehensive analysis of clinic and other radiological data.
