摘 要:目的探讨脑电图(EEG)及经颅多普勒(TCD)对病毒性脑炎早期诊断、判断病情严重程度和临床预后的应用价值。方法对75例病毒性脑炎患者发病2~15d内的脑电图和经颅多普勒结果进行回顾性分析。结果75例病毒性脑炎患者15d内脑电图异常62例,阳性率82.7%,以额颞叶最为多见。TCD异常65例,阳性率86.7%,主要表现为颈内动脉系统血流动力学的改变,大脑前动脉流速与病情严重程度成正相关。意识障碍越重,EEG和TCD的阳性率越高,异常程度也越高。结论脑电图对病毒性脑炎特别是伴有癫痫患者的早期诊断有重要价值,TCD可反映病毒性脑炎的血流动力学,在判断病情严重程度和预后方面有一定应用价值。[ Objective ] To investigate the value of the electroencephalogram (EEG) and transcranial Doppler (TCD) in early diagnosis, evaluate the severity degree and clinical prognosis of viral encephalitis. [Metheds] 75 cases with viral encephalitis within 2-15 days were reviewed. [Results] Among 75 viral encephalitis patients, 62 cases had abnormal EEG, the positive rate was 82.7%, most were in frontal and temporal lobe. 65 cases had abnormal TCD, the positive rate was 86.7%, principal representation were hemodynamic changes of internal carotid artery. The velocities of anterior cranial artery were positively correlated with the severity degrees. The more the degree of conscious disturbance increased, the higher the positive rate and the more severity degree of EEG and TCD. [ Conclusion] EEG is an important method in the early diagnosis of viral encephalitis. Transcranil Doppler reflects the cerebral blood dynamics of viral encephalitis, It has clinical value as an indicator of the severity degrees and clinical prognosis.
