摘 要:目的:探讨气腹前的预处理减轻气腹所致肝脏损伤的效果和机制。方法:SD大鼠30只,随机分为3组:假气腹组(C组,n=6)、气腹组(P组,n=12)和预处理-气腹组(PP组,n=12)。P组闭合法建立CO_2气腹,腹压为15 mmHg,维持90 min后解除气腹,PP组预处理为5 min充气和5 min放气,反复2次后建立气腹,维持90min。C组不建立气腹,麻醉时间同PP组。解除气腹后1、24 h分别取血测定血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、超氧化物歧化酶(SOD)、丙二醛(MDA),电镜观察肝组织超微结构。结果:与C组比较,P组、PP组解除气腹后1、24 h,血清ALT、AST、MDA水平显著升高(P<0.05),SOD显著降低(P<0.05)。肝组织超微结构损伤明显;PP组ALT、AST、MDA则显著低于P组(P<0.05),SOD显著大于P组(P<0.05),肝组织超微结构损伤程度减轻。结论:预处理能通过降低氧自由基水平,桔抗脂质过氧化反应,减轻气腹所致肝脏损伤。Objective To investigate the protective effects and potential mechanism of preconditioning on pneumoperitoneum-induced liver injury in rats. Methods 30 Sprague-Dawley (SD) rats were allocated randomly into 3 groups: control group (group C, n = 6), pneumoperitoneum group (group P, n = 12) and preconditioningpneumoperitoneum group (group PP, n = 12). The rats in group P were subjected to CO2 pneumoperitoneum for 90 rain with 15 mmHg intra-abdo minal pressure, followed by deflation. The rats in group PP were subjected to preconditioning, followed by 90 rain of pneumoperitoneum and deflation. Preconditioning was defned as 5 rain of pneumoperitoneum followed by 5 rain of deflation. Rats in the group C were subjected to a sham operation without pneumoperitoneum and the anesthesia time was same with that for group PP. 1 or 24 hours after deflation, blood samples were taken for detections of serum alanine a minotransferase (ALT), aspartate a minotransferase (AST), superoxide dismutase (SOD) and malonaldehyde (MDA). Liver lesions were also detected by electron microscopy. Results Compared with group C, the levels of ALT, AST and MDA were inreased significantly in group P and PP (P 〈 0.05), while the SOD levels were decreased significantly(P 〈 0.05). Compared with group P, the levels of ALT, AST and MDA in group PP decreased, and SOD levels were increased significantly (P 〈 0.05) in group PP with minor hepatic injury. Conclusion Preconditioning can attenuate pneumoperitoneum-induced hepatic injury in rats by decreasing free oxygen radieals and antagonizing lipid peroxidation.
