摘  要:目的观察针刺颈夹脊穴对大鼠退变颈椎间盘Ⅰ、Ⅱ型胶原及髓核超微结构的影响,探讨针刺治疗椎间盘退变的作用和机理。方法建立大鼠动静力失衡性颈椎间盘退变模型,随机分为对照组、针刺组、药物组,并设空白组对照,针刺组取颈部夹脊穴,药物组采用布洛芬加颈复康灌胃,治疗30d后,取部分椎间盘测Ⅰ、Ⅱ型胶原含量;取部分椎间盘在光镜和电镜下观察组织细胞学变化。结果与椎间盘退变正相关的Ⅰ型胶原含量测定,空白组少于其他各组,差异有统计学意义(P<0.05),针刺组和药物组少于模型组,差异有统计学意义(P<0.05);与椎间盘退变负相关的Ⅱ型胶原含量测定,空白组多于其他各组,差异有统计学意义(P<0.05),针刺组和药物组多于模型组,差异有统计学意义(P<0.05),其他组间比较无明显差异;光镜、电镜下椎间盘组织细胞学退变程度由轻至重顺序为空白组<针刺组<药物组<模型组;椎间盘胶原测定结果与镜下椎间盘组织细胞学退变程度相一致。结论针刺颈夹脊穴可以通过调控椎间盘细胞外基质胶原系统抑制椎间盘退变。Objective To evaluate the effect of acupuncture at cervical Huatuo jiaji on type Ⅰ and Ⅱ collagen and pulpiform nucleus ultrastructure in rat degenerate cervical intervertebral discs and explore the therapeutic action of acupuncture on intervertebral disc degeneration and its mechanism. Methods A rat model of static-dynamic imbalance cervical intervertebral disc degeneration was made. The rats were randomly allocated to model, acupuncture and medication groups. A blank group was established as a control. After 30 days of treatment, parts of the intervertebral disc were taken to measure type Ⅰ and Ⅱ collagen contents and to observe histocytologic changes under the optical microscope and electron microscope. Results The measurement of type I collagen content positively correlated to intervertebral disc degeneration showed that it was lower in the blank group than in the other groups, with a statistically significant difference (P〈0.05) and lower in the acupuncture and medication groups than in the model group, with a statistically significant difference (P〈0.05). The measurement of type II collagen content negatively correlated to intervertebral disc degeneration showed that it was higher in the blank group than in the other groups, with a statistically significant difference (P〈0.05) and higher in the acupuncture and medication groups than in the model group, with a statistically significant difference (P〈0.05). There were no statistically significant difference between the other groups. Optical and electron microscope examinations showed that the order from slight to severe histocytologic de&eneration in the intervertebral disc was the blank group 〈 the acupuncture group 〈 the medication group 〈 the model group. The measurement of intervertebral disc collagen was consistent with the microscopic degree of histocytologic degeneration in the intervertebral disc.
