2005～2007年济南市免疫接种率监测与评价

摘　　要：[目的]为了解济南市免疫接种率监测现状。[方法]采用差值(D)和比值(R)评价方法对济南市2005～2007年常规免疫接种率进行统计分析。[结果]2005～2007年"五苗"(卡介苗、新生儿乙肝疫苗、口服脊髓灰质炎疫苗、百白破联合疫苗、麻疹疫苗)常规免疫报告接种率为98.25%～99.97%,2006～2007年流脑疫苗、乙脑疫苗常规免疫报告接种率为95.04%～97.47%;估算接种率为91.82%～117.16%;"五苗"调查接种率为86.00%～96.89%。D值:以市为单位,报告接种率为可信占19个报告数据的36.84%,可疑占57.89%,不可信占5.26%。R值:共计算全市99个R值,可信占63.64%,可疑占28.29%,不可信占8.08%。[结论]近年来济南市"五苗"及两脑疫苗接种率保持在一个较高水平,但报告接种率与估算接种率、调查接种率存有一定差距,各县(市、区)D、R值及调查接种率差异,提示免疫规划工作发展不平衡。2006年估算接种率、调查接种率最低,说明《疫苗流通和预防接种管理条例》颁布后,2006年济南市常规免疫曾出现了一定下滑。[Objective]To understand the status of surveillance of routine immunization coverage in Ji＇nan city. [Methods]Data of routine immunization coverage reported from 2005 to 2007 were statistically analyzed using the evaluation method of difference （D value） and ratio （R value）. [Results]The reported coverage of routine immunization of BCG, HBV,OPV,DPT and MV was from 98.25% to 99.97% from 2005 to 2007. The reported immunization coverage of epidemic meningitis, meningitis B vaccines and estimated coverage rate of routine immunization was from 95.04% to 97.47 % and from 91.82% to 117.16% respectively from 2006 to 2007. The investigated coverage of 5 vaccines was from 86.00% to 96.89%. D value：the reliability of reported coverage accounted for 36.84% of the total 19 reported data in the city, Susceptibility accounted for 57.89% ,not reliability accounted for 5.26 %. R value： Of the 99 R value,Reliability,susceptibility and not reliability accounted for 63.64% ,28.29 % and 8.08% respectively. [Conclusion]The coverage of 5 vaccines and epidemic meningitis,meningitis B vaccines maintains a high level in Ji＇nan. However,there are some differences among the reported,estimated and investigated coverage. The difference of D,R value and the investigated coverage among these districts,and counties suggested the imbalance development of the implementation of immunization program. The estimated and investigated coverage was the lowest in 2006. It suggested that the coverage of routine immunization dropped in 2006 after ＂Regulation on Management regarding the Circulation of Vaccine and Vaccine＇s Prevention and Inoculation ＂had been issued.
