重型颅脑损伤病人标准大骨瓣减压的临床治疗体会

摘　　要：目的：探讨标准外伤大骨瓣减压术治疗重型颅脑损伤的临床效果。方法：对58例重型颅脑损伤患者均采用标准外伤大骨瓣开颅减压术治疗并观察其治疗效果。结果：6个月按照GOS分级方法进行预后评估：恢复良好（中残）25例，重残13例，持续性植物状态9例，死亡11例。结论：标准外伤大骨瓣减压术比常规骨瓣减压术可使脑组织得到充分减压，有效保护脑功能，明显改善重型颅脑损伤患者的预后。Objectiv ： to explore the efficacy of large standard trauma craniotomy for severe traumatic brain injured patients. Methods ： standard large trauma craniectomy was performed in 58 cases with severe traumatic brain injured, and observe it＇s therapeutic efficacy. Result ： After 6 months follow up,25 cases recov- ered well, 13 cases severely disabled,9 cases was under vegetative state and 11 cases died. Conclusion：The standard large trauma craniotomy reduced intracranial pressure effectively and protects the function of brain tissue. The prognosis of severe traumatic brain injured patients could be improved by this technique.
