基层医院腹腔镜胆囊切除术820例临床分析

摘　　要：目的:总结基层医院开展腹腔镜胆囊切除术(laparoscop ic cholecystectomy,LC)的手术经验,并探讨并发症的预防措施。方法:回顾分析我院为820例患者施行LC的临床资料。结果:腹腔镜下成功完成胆囊切除术808例(98.5%),12例中转开腹。术中十二指肠损伤1例(0.1%),胆漏2例(0.2%),出血2例(0.2%),无死亡病例。结论:合理掌握手术适应证,把握中转开腹时机,规范手术操作,基层医院开展LC安全可行,可作为基层医院治疗胆囊良性疾病的"金标准"。Objective：To summarize operative experience of laparoscopic cholecystectomy（LC） and explore prevention of complication in grass-roots hospital.Methods：The clinical data of 820 patients performed LC in our hospital between Aug.2002 and Oct.2008 were retrospectively analyzed.Results：LC succeeded in 808 cases（98.5%）.12 cases were converted to open cholecystectomy.Duodenal injury happened in 1 patient（0.1%）.Bile leakage was found in 2 patients（0.2%）.No death occurred.2 cases suffered from postoperative bleeding（0.2%）.No death occurred.Conclusions：It is safe and feasible to perform LC in grass-roots hospital with appropriate indication,standard performation and reasonable opportunity of conversion as a gold standard surgical procedure for cholecystic benign diseases.
