7例主动脉根部加宽主动脉瓣替换术临床分析

摘　　要：目的探讨狭小的主动脉根部行主动脉瓣机械瓣替代术时主动脉根部加宽的方法和结果。方法6例机械瓣双瓣置换(DVR),1例单纯主动脉瓣置换(AVR)患者主动脉根部狭小行主动脉根部加宽(Manouguian法为主)手术的临床资料进行分析。结果7例主动脉根部瓣环扩大直径5～8mm,主动脉根部植入21～23人工机械瓣,手术死亡2例,存活5例随访2～5年心功能良好。结论主动脉根部加宽采用Manouguian法对小主动脉小瓣环和冠状动脉开口下移的患者是安全有效的,此法可以根据术中情况酌情扩大主动脉根部。Objective To explore the surgery procedure and its result of enlargement annulus for aortic valve replacement in small root.Methods There were 7 patients with small root undergoing double valve replacement（DVR） for 6 patients and aortic valve replacement（AVR） for 1 case.The clinical data were analyzed.Results The diameters of aortic annulus after operation were enlarged to 5-8 mm.The prosthetic valve（Φ 21-23） were implanted in aortic valve position.Postoperative death occurred in 2 cases.Follow-up of 2 to 5 years,the heart function of 5 survival patients was excellent.Conclusion The Surgical procedure（with Manouquian＇s Method） was safe and effective for valve replacemental patient of small aortic root and lower site ostia of coronary.
