个性化自制匀浆膳对管饲患者营养支持的临床应用

摘　　要：目的观察自制匀浆膳对管饲患者营养支持的临床效果。方法选择65例神经内外科的昏迷患者,采用匀浆膳食进行营养支持治疗。监测使用前后淋巴细胞总数、血红蛋白、血清清蛋白、前清蛋白和肱三头肌皮皱厚度、臂围、臂肌围等,并统计使用费用。结果本组患者平均使用匀浆膳时间为(19.2±19.9)d,提供的热卡平均为(1347.1±232.2)kCal,所耗费用为(64.9±10.8)元,与同类商品化肠内营养制剂相比,在提供等热卡的情况下可节省费用30元。所有患者无严重胃肠道不良反应。患者的营养状况在第14天明显改善,淋巴细胞总数、血清蛋白、血清前清蛋白较治疗前均有明显提高,使用前后肝肾功能无异常。结论匀浆膳是管饲患者营养支持的一种较理想的制剂,可降低患者同类营养支持费用,并能有效保证患者的营养需要。Objective To observe the effect of home-made homogenate meals nutritional support in tube feeding patients. Methods Sixty-five comatose inpatients of department of neurology medico-ehirurgical were fed with home-made homogenate meals per naso-gastric tube. The total lymphocyte count, hemoglobin, serum albumin, prealbum and triceps skinfold thickness, mild arm circumference and arm muscle circumference were measured. The cost of nutrition liquid were analysed. Results The average time of usage home-made homogenate meals was （19.2 ±19.9 ）days, the average calore was （1347.1 ±232.2）kCal, the average cost was ￥ （64.9±10. 8）,less ￥30 than the same kind of commercialization nutrition product. No severe adverse effects in gastrointestinal,The states of nutrition in 14^th（ such as lymph cells, hemoglobin, serum levels of albumin, pre-album） were improved significantly after home-made homogenate meals, no any adverse in liver and renal function. Conclusion The homoe-mate homogenate meals can support the comatose patients effectively and lower the cost.
