自制双腔T形管在胆道术后施行肠内营养中应用的效果

摘　　要：目的探讨自制双腔T形管在胆道疾病患者术后施行肠内营养中的应用效果。方法将75例胆道疾病术后患者按入院先后分为两组,使用自制双腔T形管行肠内营养39例为实验组,使用常规空肠置营养管行肠内营养36例为对照组。对两组患者并发症发生情况、肠内营养使用量及营养过渡至TEN所需时间、肛门排气时间、营养相关指标进行比较,同时观察T形管引流情况。结果两组患者在腹痛、腹胀、腹泻、营养管堵塞、胆瘘等并发症发生率方面,差异无统计学意义(P>0.05)。两组患者在达到肠内营养每天最大使用量及营养过渡至TEN所需时间、肛门排气时间、术后相关营养指标方面的差异亦无统计学意义(P>0.05)。实验组患者手术时间短、住院费用低,与对照组患者相比差异有统计学意义(P<0.05)。结论自制双腔T形管具有引流、冲洗、肠内营养及留置支架等多种用途,操作简便,行肠内营养安全可行。Objective To explore the application effects of the enteral nutrition administered through self-made double lumen T tube after biliary tract surgery. Methods Seventy-five patients undergoing biliary tract surgery were divided into the experimental group （n=39） and the control group （n = 36） according to the admission sequence. The patients in the experimental group received self-made double lumen T tube and the patients in the control group received enteral nutrition through the jejunum canal. The incidence of complications, the maximum quantum of the enteral nutrition, the course of recovery of the peristaltic movement of the intestinal tract, the index related to the patients nutrition status and the drainage of the T tube were investigated. Results There were no significant differences in the incidence of abdominal distension,diarrhea, blockage of nutrient canal,biliary fistula （P〉0.05）. No significant differences were observed in the time of reaching the maximum quantum of the enteral nutrition,recovery course of passing the gas and the index related to the patients nutritional status after surgery（P〉0.05）. The operating time and cost for the experimental group were significantly less than those for the control group （P〈0.05）. Conclusions It is a safe and feasible method through self-made double lumen T tube in patients undergoing biliary tract disease for drainage,washing,enteral nutrition and replacing framework.
