肝阳上亢证实验诊断指标研究

摘　　要：采用异病同证、同病异证、相关证对照的方法,选择血浆去甲肾上腺素(NE)、肾上腺素(E)、大脑中动脉平均血流速度(McA—Vm)及收缩峰血流速度(Vs)作为研究肝阳上亢证的实验诊断指标。结果发现,肝阳上亢证上述4项指标的测值均显著高于健康人组、肝肾阴虚证及气虚血瘀证组,肝阳上亢证组MCA—Vm、Vs测值显著高于肝火上炎证、肝阳化风证组;高血压病的肝阳上亢证与同病异证组(肝火上炎、肝肾阴虚证组)之间4项指标比较,肝阳上亢证组的血浆NE、E显著高于肝肾阴虚证组,而MCA—Vm、Vs测值显著高于肝火上炎、肝肾阴虚证组。肝阳上亢证组血浆NE与E,MCA—Vm与Vs,NE与Vs之间存在正相关系。提示上述4项指标可考虑作为肝阳上亢证的辨证诊断综合指标。By adopting the method of different disease of a same syndrome, different syndromes of a same disease, and correlative syndromes as control, indices of contents of serum noradrenalin (NE), adrenaline (E), average blood circulatory velocity (Vm) of middle cerebral artery (MCA), and velocity of blood flow at systolic peak (Vs) were selected for studying the syndrome of hyperactivity of liver-yang. Results revealed that the values of all 4 indices were higher than those of normal person and groups of asthenic syndrome of liver and kidney yin, and deficient - Qi - static - blood syndrome. MCA - Vm, Vs values were markedly higher than those in flaring - up of liver fire syndrome, syndrome of evil wind transformed from liver yang. Comparison of 4 indices between hyeractivity of liver yang due to hypertension, and different syndromes of the same disease (including flaring - up of liver fire, yin - deficiency of river -kidney) showed that, in the hyperactivity of liver yang syndrome, the serum NE, E were all markedly higher than syndrome of deficiency yin of liver - kidney; while MCA -Vm, Vs were markedly higher than that of the syndrome of flaring -up of liver fire and yin -deficiency of liver -kidney. In hyperactivity of liver -yang syndrome, there was a positive correlation NE and E, MCA - Vm and Vs, NE and Vs, demonstrating that the 4 indices can be served as a comprehensive index for diagnosis of hyperactivity of liver yang syndrome.
