耳鸣严重程度评估方法的研究

摘　　要：目的探讨合理评估耳鸣严重程度的方法。方法对219例初诊耳鸣患者分别应用耳鸣程度评分法及THI(tinnitus handicap inventory,THI)评分法进行严重程度评估,对两种方法评估的总分及其子项目评分进行相关性分析。结果耳鸣程度评分法与THI评分法的相关系数为0.73(P<0.001),按耳鸣程度评分法进行的严重程度分级与THI评分呈正相关,耳鸣程度评分法中耳鸣对情绪的影响及耳鸣对工作和生活、睡眠的影响分别与THI评分中的情绪性评分及功能性评分具有较高的相关性(相关系数分别为0.674、0.703)。耳鸣程度评分法中耳鸣对情绪、工作和生活、睡眠的影响及严重性自我评分4个子项目与总分的相关性较大,耳鸣出现的环境及持续时间2个子项目与总分的相关性较小。结论耳鸣程度评分法与THI评分法的评估理念基本一致,结果相似,但前者更为简单实用,今后可进一步改良部分指标的权重。Objective To search for appropriate methods for assessing tinnitus gravity. Methods The Tinnitus Severity Score （TSS） and the Tinnitus Handicap Index （THI） were administered in 219 patients who were diagnosed with tinnitus. The correlation between the two scoring systems were analyzed. Results The correlation coefficient between the TSS and THI was 0.73（P 0.001）. The degree of tinnitus gravity measured by the TSS was positively correlated with THI scores. The subcategories regarding influence of tinnitus on mood,work,life as well as sleep were highly correlated to the emotional and functional assessment scores in THI （r = 0.674 and 0.703 respectively）. TSS scores on influence of tinnitus on mood,sleep,work and life,and self-evaluated gravity were highly correlated to the total score. The environment in which tinnitus occurred and the time course of tinnitus were loosely correlated to the total score. Conclusion The TSS and THI share similar approach to tinnitus gravity evaluation and yield similar results. However,the former is easier to administer compared to the latter,although it can be improved by modifying some weighting factors within the system.
