实验性脑出血后水孔蛋白-4水平的变化及阿加曲班的干预作用

摘　　要：目的探讨脑出血后(ICH)血肿周围组织水孔蛋白-4(AQP4)含量的变化,以及早期抗凝血酶治疗对其表达的影响。方法采用未抗凝新鲜自体股动脉血注入大鼠尾状核建立脑出血动物模型。将60只大鼠随机分成对照组、脑出血组、抗凝血酶(阿加曲班)组,各实验组分6h、1d、3d和7d共4个时间点。分别采用干重湿重法、免疫组织化学法,测定不同时间点的脑组织水含量和AQP4表达的变化。结果脑组织水含量和AQP4表达均在ICH后6h开始升高,3d达到高峰,7d下降至正常水平,两者之间存在明显的相关性(r=0.81,P<0.01)。抗凝血酶治疗能够降低各个时间点的脑组织水含量和AQP4的表达。结论AQP4与脑水肿的形成密切相关;抗凝血酶治疗可以降低脑出后血肿周围脑组织AQP4的活性,减轻ICH后的脑水肿形成。[ Objectives] To investigate dynamic changes of water content and aquaporin-4 （AQP4） in intracerebral hemorrhagic （ICH） rats and the role that AQP4 plays in cerebral edema after hemorrhage, and to evaluate the effect of Argatroban in reducing the content of cerebral edema and AQP4. [ Methods ] Animal model was made by injecting self arterial blood into the caudate nucleus of rats. 60 rats were randoinly assigned to control group cerebral hemorrhage group and Argatroban treated group. After the injection, the rats were sacrificed at 6 h, I d, 3 d, 7 d respectively. Brain water content was determined by wet-dry weight, as the expression of AQP4 was detected by immunohistochemical method over a time course ranging from 6 h to 7 d. [Results] Brain water content and AQP4 increased after 6h of cerebral hemorrhage and reached its climax at the 3d, then decreased to the normal level at the 7 d. And there is an obvious correlation between them （r =0.815, P 〈0.01）. Administration of Argatroban could reduce the content of brain edema and AQP4. [Conclusion] Argatroban can reduce AQP4 expression, thus can reduce cerebral edema after ICH. These suggest a close correlation between AQP4 and cerebral edema, Argatroban might be available for the therapy of cerebral edema after ICH.
