鼻内镜下聚焦超声治疗下鼻甲肥大的疗效评价

摘　　要：目的探讨鼻内镜下聚焦超声治疗下鼻甲肥大的临床应用价值。方法选择65例下鼻甲肥大患者,随机分为聚焦超声组(35例)和下鼻甲部分切除术(PIT)组(30例),观察聚焦超声与PTT对下鼻甲肥大治疗的临床预后,评价其临床疗效。结果聚焦超声组35例中,治愈率2.9%(1/35),显效率60%(21/35),无效率37.1%(13/35),总有效率为62.9%,术后无一例发生手术并发症。PIT组30例中,治愈率33.3%(10/30),显效率60%(18/30),无效率6.7%(2/30),总有效率为93.3%,术后并发症发生率为13.3%(4/30)。结论鼻内镜下聚焦超声治疗下鼻甲肥大具有操作简单、创伤小、可重复性等其他术式无法比拟的优点,安全性较高;但治疗有效率低于治疗下鼻甲肥大的其他术式。Objective To evaluate the clinical efficacy and safety of high intensity focal uhrosound （HIFU） in treatment of inferior turbinate hypertrophy. Methods Sixty - five patients with inferior turbinate hypertrophy were divided into two groups randomly. One group （35 cases） was treated by HIFU;the other group （30 cases） by partial inferior turbinectomy （PIT）. All the 65 patients were followed up and the efficacy and safety of HIFU and PIT were evaluated within 6 months after the treatment. Results Of the 35 patients in HIFU group,1 （2.9%） achieved significant therapcutic effects; 21 （60%） moderate therapeut effects and 13 （37.1%） no effects, with a total effective rate of 62.9% ; no complication occurred. Of the 30 patients in PIT group, 10 （ 33.3% ） achieved significant therapeutic effects; 18 （60%） moderate curative effects and 2 （6.7%） no effects, with a total effective rate of 93.3 % ;four （ 13.3% ） had complicatioas. Conclusions HIFU therapy is a safe and effective method for treating inferior turbinate hypertrophy;but its effectiveness is lower than those of other operations.
