摘 要:目的探讨正常脊髓动脉血管及脊髓血管畸形的64排螺旋CT成像方法的优化及其临床应用价值。方法收集135例CT增强扫描受检者,其中15例MRA提示有脊髓血管畸形;应用GE LightSpeed 64排CT,采用不同的CT增强扫描触发阈值及延时扫描时间,显示正常脊髓动脉血管及畸形血管的解剖细节。脊髓血管畸形均经DSA证实。结果最佳扫描参数为:扫描速度0.4s/转、层厚5 mm、螺距0.5;增强扫描触发阈值180 Hu;达到该阈值后再延迟19 s后自动启动扫描;原始数据按0.625mm层厚、0.5螺距、Std算法进行横断面图像重建;采用优化的扫描参数和恰当的图像重建技术,MSCTA能够清晰显示肋间动脉或腰动脉、前根髓动脉、脊髓前动脉、血管畸形的供血动脉、畸形血管团及引流静脉;脊髓后动脉显示不佳;同时脊髓动脉、畸形血管的起止、各分支及走行特点也能得到很好显示。结论64排CTA可作为无创性脊髓血管成像的首选检查方法。Objective To study the visualization of normal spinal artery in volunteers and abnormal spinal vascularity in patients with spinal vascular malformations(SVM) using 64 - slice CT angiography. Methods CT angiography was performed in 120 volunteers and 15 patients with the symptoms of spinal vascular malformations proved by MRA. Different trigger thresholds of CT scanning and delay times were used to manifest anatomical details of spinal artery and spinal vascular malformations with MPR, CPR and MIP. SVM were confirmed by DSA in 15 patients. Results The best scan parameters are as follows: scan speed, 0.4s; slice reconstrucion thickness, 0.625 mm; pitch, 0.5; trigger threshold of CT scanning, 180Hu, and delay time 19s. Conclusions with optimized scanning parameters and appropriate image reconstruction technology, we can precisely identify intercostal arteries, lumbar arteries, Adamkiewitz artery, anterior spinal arteries, and vascular malformations including feeding arteries and draining veins. 64 - slice CT angiography is the preferred non - invasive imaging procedure in spinal vascular diseases.
