反复自然流产患者封闭抗体检测的临床意义

摘　　要：目的研究原因不明反复自然流产(RSA)患者中封闭抗体检测的临床意义,探讨原发反复自然流产和继发反复自然流产患者中封闭抗体阴性率的差异。方法采用酶联免疫吸附试验(ELISA)检测279例原发性反复自然流产患者、147例继发性反复自然流产患者和60例正常孕妇血清的封闭抗体。结果反复自然流产患者封闭抗体阴性率为82.63%,明显高于对照组的阴性率13.3%,差异有统计学意义(P<0.05)。原发反复自然流产患者封闭抗体的阴性率为99.28%,继发反复自然流产患者封闭抗体阴性率为51.02%,两者差异有统计学意义(P<0.05)。结论反复自然流产与患者封闭抗体缺乏有关,其中封闭抗体缺失在原发性反复自然流产患者中更为明显,在流产患者中及时进行封闭抗体的检测,对于RSA的实验室诊断及免疫治疗非常重要。Objective To study the clinical significance of antipaternal lymphocyte antibody （APLA） in patients with recurrent spontaneous abortion, and to explore the difference of APLA＇s negative rate between primary and secondary recurrent spontaneous abortion patients. Methods APLA was detected by enzyme - linked immunosorbent assay （ELISA） in 2？9 patients with primary recurrent spontaneous abortion, 147 patients with secondary recurrent spontaneous abortion, and 60 normal controls. Results The negative rate of APLA in patients with recurrent spontaneous abortion （82. 6396 ） was significantly higher than that of controls （13.3 % ）, and the difference was statistically significant （P〈 0.05）. The negative rate of APLA in patients with primary recurrent spontaneous abortion （99.28 % ） was significantly higher than that of patients with secondary recurrent spontaneous abortion （51.02%）, there was a statistically significant difference between them （P〈 0.05）. Conclusions There is some relationship between recurrent spontaneous abortion and the lack of APLA, the deficiency of APLA in patients with primary recurrent spontaneous abortion is more significant. It is very important to timely detect ALPA for laboratory diagnosis and immunotherapy of abortion patients.
