乌司他丁对人凝血功能影响的体外观察

摘　　要：目的探讨不同浓度乌司他丁对体外全血凝血功能的影响。方法取16例健康成年志愿者静脉血,在体外与不同浓度的乌司他丁混合,使其终浓度分别为25、50、75、100、200U/ml(n=16),采用Sonoclot凝血及血小板功能分析仪进行凝血功能检测,指标包括激活的凝血时间(ACT)、凝结速率(CR)、血小板功能(PF)等。以未经处理的全血作为对照。结果25、50、75U/ml组ACT与对照组比较无显著差异(P>0.05),而100、200U/ml组ACT较对照组显著延长(P<0.05);各组CR与对照组比较差异均无统计学意义(P>0.05);25、50U/ml组PF与对照组比较无显著差异(P>0.05),而75、100、200U/ml组PF均显著低于对照组(P<0.05)。结论乌司他丁在全血中的浓度达到100U/ml以上时,能有效地延迟内源性凝血通路的活化,并在一定程度上抑制血小板功能,从而延缓病理性凝血过程的出现,改善大型手术围术期凝血功能。Objective To investigate the effects of ulinastatin on blood coagulation in vitro. Methods Ulinastatin was mixed with venous blood samples obtained from 16 healthy adult volunteers to reach the plasma concentration of 25U/ml, 50U/ml, 75U/ml, 100U/ml and 200U/ml, respectively. Coagulation function was measured with Sonoclot coagulation and platelet function analyzer. The parameters measured included activated clotting time （ACT）, clot rate （CR） and platelet function （PF）. Blood sample without ulinastatin treatment was taken as control group. Results Compared with control value, no significant difference of ACT was observed in 25U/ml, 50U/ml and 75U/ml of ulinastatin treated groups （P〉0. 05）, while in 100U/ml and 200U/ml groups, ACT was significantly prolonged （P〈0.05）. Compared with control value, no significant difference of CR was found in all groups （P〉0.05）. Similarly, PF in 25U/ml and 50U/ml groups showed no significant difference compared with that in control group （P〉0. 05）, while in 75U/ml, 100U/ml and 200U/ml groups, PF decreased significantly （P〈0.05）. Conclusion Ulinastatin may effectively delay the activation of intrinsic coagulation pathway when the dosage reaches 100U/ml and higher, and it may inhibit platelet function in certain extent, thereby postpone pathological process of clotting, ameliorate undue coagulation of blood after major operations.
