阿托伐他汀强化降脂对高血压病患者颈动脉粥样硬化的影响

摘　　要：目的观察合并有颈动脉粥样硬化的高血压病患者阿托伐他汀强化降脂后颈动脉内膜中膜厚度(CIMT)的变化。方法选择伴有不同程度的颈动脉粥样硬化的高血压病患者126例,随机分为2组。A组(强化治疗组)65例,服用阿托伐他汀40mg,每晚1次;B组(常规治疗组)61例,服用阿托伐他汀10mg,每晚1次,疗程6个月。观察治疗前后CIMT以及总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)的变化。结果阿托伐他汀40mg、10mg(P<0.05)、TC、LDL-C(P<0.05)和升高HDL-C水平(P<0.05)。结论阿托伐他汀强化降脂能更有效地降低CIMT,以及TC和LDL-C水平,并具有良好的安全性。Objective To investigate the effect of intensive lipid lowering of atorvastain on carotid intima-media thickness（CIMT） in patients with hypertension.Methods Total 126 hypertensive patients with carotid atheroma were randomized to atorvastatin 40mg/day A group （n=65） and 10mg/day B group （n=61） for six months.The carotid artery inner-intermediate thickness（CIMT） and the speckle’s size were examined by the color Doppler,and blood lipids,liver enzyme and muscle enzyme were detected.Results After six months of treatment, CIMT was obviously decreased in both atorvastatin 40mg/day A group and 10mg/day B group, which could be further decreased in A group than B group. The levels of TC, and LDL - C in both groups were decreased after treatment. Furthermore, the changes in A group were more obvious than in B group. No more side effect was found in 40mg/day atorvastatin group. Conclusion Intensive treatment by atorvastain can more obviously decrease TC and LDL- C and prevent the progress of carotid atheroma in hypertension patients safely.
