江苏地区2型糖尿病与亚临床甲减发生率关系调查分析

摘　　要：目的研究亚临床甲减(亚甲减)与2型糖尿病的关系。方法2006年10月至2007年6月采取逐级分层随机抽样方法抽取江苏地区6个地区≥20岁的长住(≥5年)2型糖尿病患者416例,采集空腹静脉血标本,以固相化学发光酶免疫分析法(ICMA)检测甲状腺功能。结果(1)2型糖尿病合并亚甲减的发生率为12.02%,占糖尿病患者中甲状腺疾病的52.63%,明显高于其他甲状腺疾病(P<0.01);(2)与甲状腺功能正常的糖尿病患者相比,糖尿病患者中亚甲减患者年龄更大,糖尿病病程更长(P<0.01);(3)亚甲减组FT4明显低于甲状腺功能正常组[分别为(16.26±2.51)pmol/L和(17.56±2.60)pmol/L],差异有统计学意义(P<0.01)。结论2型糖尿病合并亚甲减较为常见,受患者患者年龄和糖尿病病程影响,加强对糖尿病患者中亚甲减的筛查和早期诊治具有重要的临床意义。Objective To explore the correlation between subclinical hypothyroidism and type 2 diabetes. Methods Diabetic patients who had lived for at least five years and aged more than 20 yeasrs old were sampled from six layers in Jiangsu Province by a stratified cluster sampling technique,Serum was sampled from 416 subjects and serum thyroid function was measured by chemiluminesecence. Results （ 1 ）The prevalence rate of subclinical hypothyroidism was 12. 21% , the proportion of subclinical hypothyroidism was 52.63% in all diabetic combined with thyroid disease ,which was significantly higher than those of other thyroid diseases （P 〈 0. 01 ）. （2）Compared with euthyroid diabetic patients, subclinical hypothyroidism patients was much older,and the diabetes duration was significantly longer（ P 〈 0.01 ）. 3. Serum FT4 level in subclinical hypothyroidism group was significantly lower than that of euthyroid group in diabetes [ （ 16. 26 ± 2. 51 ） pmol/L vs （ 17.56 ± 2.60）pmol/L, P 〈 0. 01 ]. Conclusion It is common that subclinical hypothyroidism combined with diabetes in diabetic patients in Jiangsu Province,which may be influenced by age and diabetes duration, and more attention should be paid to the follow-up and diagnosis of subclinical thyroid disease in diabetic patients.
