40例手辅式腹腔镜左半肝切除术的临床疗效分析

摘　　要：目的分析手辅式腹腔镜左半肝切除术的临床疗效。方法回顾分析40例行手辅式腹腔镜左半肝切除术患者与44例同期行开腹左半肝切除术患者的手术效果。结果手辅式腹腔镜组在术中出血量、术后体温恢复、术后下床活动、术后排气、住院时间、术后并发症、术后对肝功能影响等方面与开腹组相比具有明显的优点,差异有统计学意义(P<0.01)。结论手辅式腹腔镜左半肝切除术是一种安全、有效的手术方式,有较高的临床应用价值。Objective To analyze the feasibility of hand-assisted laparoscopic left liver resection. Methods The data on the 40 patients undergoing hand-assisted laparoseopic left liver resection and 44 undergoing transabdominal left liver resection in the same period were reviewed respectively,and were compared statistically. Results Compared with patients undergoing transabdominal left liver resection, the patients undergoing hand assisted laparoscopic left liver resection had statistical differences in diminished intraoperative bleeding, rapid recovery of body temperature and restoreation of off-bed activity after operation, rapid postoperative anal aerofluxus, shortening postoperative hospital stay, reducing postoperative complications and reducing postoperative ALT level （P〈 0.01）. Conclusion Hand-assisted laparoscopic left liver resection is safe and effective, which has many advantages compared with the conventional transabdominal left liver resection and is valuable to clinical application.
