摘 要:目的:观察黄芪注射液对大鼠肾脏缺血再灌注细胞凋亡的影响,探讨其对肾脏缺血再灌注损伤的保护机制。方法:将实验动物分成假手术组、肾脏缺血再灌注组、黄芪组。测定各组血清尿素氮(BUN)、血清肌酐(Scr)水平及肾组织丙二醛(MDA)水平,超氧化物歧化酶(SOD)、乳酸脱氢酶(LDH)、氧化亚氮(NO)、氧化亚氮合酶(NOS)活性;HE染色观察肾组织病理变化情况,并进行中性粒细胞(PMN)计数;TUNEL法观察肾组织细胞凋亡情况。结果:黄芪组血清BUN,Scr水平,肾组织匀浆MDA水平、LDH活性较缺血再灌注组显著下降,SOD,NO,NOS活性较缺血再灌注组显著升高;肾组织切片镜下显示缺血再灌注组肾小管大量坏死、变性、管内扩张出血,部分肾小球内亦出现红细胞,黄芪组肾小管、肾小球病变减轻较为明显;中性粒细胞(PMN)数黄芪组较缺血再灌注组显著减少;TUNEL法测定肾小管上皮细胞阳性凋亡率黄芪组较缺血再灌注组明显减少。结论:黄芪可以通过增加肾组织NO水平,减少氧自由基,抑制细胞凋亡而减轻肾脏缺血再灌注损伤。Objective: To observe the influence of Astragalus injection on the apoptosis of cells in rats after renal ischemia reperfusion injury in order to elucidate the mechanism of its protective effect.Methods: Forty-eight male SD rats were divided randomly into three groups: the sham operated-group,the renal ischemia reperfusion(RIR) group and the Astragalus injection preconditioning group.The levels of serum creatinine(Scr),blood urea nitrogen(BUN),the activity of renal superoxide dismutase(SOD),lactic dehydrogenase-H(LDH),NO,nitric oxide synthase(NOS) and the content of malondialdehyde(MDA) were detected respectively.The pathological changes of kidney tissue were investigated by hematoxylin-eosin(HE) staining,the number of PMN was counted at the same time.Cell apoptotic rate was examined by method of transferase-mediated d-UTP nick end labeling(TUNEL). Results: Both the levels of Scr,BUN,MDA and the activity of LDH were significantly lower,while the activities of SOD,NO and NOS were dramatically higher than those in RIR group.There were significant pathological changes in renal tissue in RIR group: degeneration,necrosis and congestion were showed in many renal tubules,besides,there were erythrocytes in some glomerulus.However,fewer injuries were found in Astragalus group.The number of PMN was very high in RIR group while it significantly decreased in Astragalus group.TUNEL analysis showed that the apoptotic rate of tubular epithelial cell in Astragalus group was apparently lower than that of RIR group. Conclusion: Astragalus attenuated renal ischemia reperfusion injury by increasing the level of NO,decreasing the oxyradical,and therefore inhibiting the apoptosis of cells.
