间断两疗程法应用GM1治疗中重度新生儿缺氧缺血性脑病的临床观察

摘　　要：目的观察间断两疗程法应用单唾液酸四己糖神经节苷脂(GM1)早期治疗中重度新生儿缺氧缺血性脑病(HIE)的疗效。方法中重度HIE患儿92例随机分为3组:在常规治疗的基础上,对照组(30例):加用胞二磷胆碱0.125g/次,1次/天,10天为一疗程;干预1组(31例):应用GM1 20 mg静脉滴注治疗,1次/天,10天为1个疗程;干预2组(31例):应用GM1 20 mg静脉滴注治疗,1次/天,10天为1个疗程;休息7-10天,进行第2疗程治疗,即间断应用两疗程。3组分别检查头颅CT;行为神经评分(NBNA);并于生后1个月起至生后12个月每月检测一次精神运动发育商(DQ),采用首都儿科研究所修订的儿心量表测定。统计学采用SPSS13.0软件,多个样本均数的比较采用单因素方差分析。结果干预组头颅CT影像学脑结构的恢复,NBNA评分,DQ值明显优于对照组;且干预2组DQ值在6月龄,12月龄明显优于干预1组。结论间断两疗程法应用GM1早期治疗中重度新生儿HIE能更好保护脑功能,改善远期疗效。Objective： To study the effects of GM1 in the brain tissue of moderate or severe neonatal HIE with two courses and early treatment.Methods： The moderate or severe neonatal HIE 92 cases were divided into three groups：all the patients were based on the the conventional treatment,besides this the control group（30 cases）,were treated by the CDP-choline,0.125 g per time;the intervention group 1（31cases）,applied GM1（20mg） per day,intravenous drip for treatment within 48 hours after the birth,10 days for a course; the intervention group 2 （ 31 cases）, applied GMI （20rag） per day, intravenous drip for treatment within 48 hours after the birth, 10 days for a course. Rest for 7 - 10 days, then give the second course of treatment, that means treated by the method of GM1 intermittent for two courses. Examination of CT for brain, NBNA;Testing the DQ monthly for one year, adopted the CDCC amendment by the Capital Institute for Pediatric. SPSSI3.0 software were used in this study. Results：The head CT imaging , NBNA scores and DQ value in the intervention group were better than control group. Conclusion： The method of treatment with GMI intermittent for two courses and early treatment could protect the brain function better, improve the long-term efficacv.
