乙肝病毒血清标志物与DNA定量联合检测的临床应用价值

摘  要:目的探讨乙型肝炎病毒(HBV)两对半抗原抗体系统、前S1蛋白(Pre-S1)和HBV脱氧核糖核酸(HBV-DNA)之间的关系及临床意义。方法用酶联免疫吸附试验(ELISA)检测血清标志物,用荧光定量PCR检测HBV-DNA。结果A组(HBsAg+HBeAg+抗HBc+)HBV-DNA、Pre-S1阳性率为98.63%、86.30%;B组(HBsAg+抗HBe+抗HBc+)HBV-DNA、Pre-S1阳性率为48.37%、22.22%;C组(其它模式)HBV-DNA、Pre-S1阳性率为4.96%、0.83%。HBV-DNA定量结果A组显著高于B、C两组(P<0.01);B组显著高于C组(P<0.05)。结论正确合理地分析HBV血清学指标出现的类型,与HBV-DNA的联合动态检测有助于临床诊断、疗效观察及预后判断。Objective To explore the internal relations between expressional patterns of serological markers of hepatitis B virus (HBV) and HBV DNA and to access its clinical value. Methods HBV serum markers and HBV DNA quantity were detected by ELISA and fluorescence quantitative PCR. Results The positive rates of HBV DNA and Pre-S1 was significantly different among three groups (P〈 0. 01). The HBV DNA content in group A(HBsAg^+ , HBeAg^+ , anti- HBc^+ ) was higher than that in group B (HBsAg^+ , anti-HBe^+, anti-HBc^+) and group C (P 〈0.01). Compared with group C (other than group A and B), there was a higher HBV DNA content in group B(P〈0.05). Conclusions Correlation analysis of different patterns of HBV serum markers and HBV DNA is helpful to clinical diagnosis, treatment and monitor for the state of HBV infection.
